REVIEW: The Zoning Administrator will review the application for completeness and assign a file number to the application. The Zoning Administrator may require additional information and will notify the applicant of this within 10 days. Upon receiving a complete application and supporting documents, the Zoning Administrator will: • send copies of the applications to the appropriate reviewing agencies for comment. Applicants are encouraged to contact their town to discuss their application and inquire about necessary building permits and approvals at the town level; • schedule a site visit to the applicant's property, at which time the applicant shall flag all applicable property/project corners and label the flags accordingly; and • review the file and prepare findings for approval or denial of the permit within 60 days. Upon approval, the permit will be mailed to the applicant or to the applicant's agent. If approved, the land use permit will be valid for one year from the date of the permit issuance. The applicant may also be required to apply through the Town for a local building permit. All site plans, pictures, and other materials submitted with the application become the property of the Planning and Zoning Department and will remain in the file. G Code Administma f Parcel # 14.30.18.209A, Town of Richmond 715-386-4680 Land Information Dear Mr. Hall: Planning 715-386-4674 yt 0v~ This letter confirms zoning approval according to the plans you have submitted to the Planning and Zoning Department to co-locate AT&T equipment at an existing wireless Real P communication tower located on property listed above, owned by Steven J. Derrick in the 715, -4677 Town of Richmond. Staff finds that the project meets the tower provisions in Sections R cling 17.85(2) 17.86 (4) (5) and (6) and 17.87(2) and (4) of the St. Croix County Zoning Ordinance 5-386-4675 based on the following fmdings: 1. Approval of this land use permit includes the addition of new AT&T facilities to an existing 300 tower on property leased to American Tower Corporation, including placement of antennas at a centerline height of 160 feet and construction of a new 12' x 20' equipment shelter. 2. A preliminary landscape plan has been submitted along with a financial surety for the planting of (24) 6 foot tall evergreen trees to screen the existing 60 foot by 60 foot fenced area. The surety is for 120% of the estimated amount to plant the trees. 3. The antenna will remain gray in color to blend in with the existing tower. 4. All proposed AT&T antennae and equipment meets the required setbacks and height requirements in the Ag-Residential District. 5. The existing tower has the capacity to structurally support the proposed AT&T facilities as certified by a registered professional engineer for American Tower .:r Engineering Services in a report dated February 2, 2009. 6. The proposed AT&T facilities meet mechanical, structural, and electrical standards as indicated in Sheets C-7 and E-1 through E-4 of the specification sheets dated February 4, 2009 and submitted as part of this permit application. 7. The proposed AT&T facilities will provide cellular and Personal Communication Services (PCS) that will not interfere with other existing facilities or pose a threat to .Y public health and safet y as certified by Jason Hall The Hall Institute, in a letter dated February 24, 2009. 8. The applicant shall contact the Planning and Zoning Department at least five days prior to commencing installation of the proposed antennae and facilities. 2. The applicant must install and maintain erosion control measures required during construction as stated in General Notes items 26 and 27 on page GN-1. 3. The applicant shall inform the Zoning Administrator when construction is complete and submit photos of the completed antennae and facilities. ~4. The applicant shall submit a revised landscape plan that shows the screening around the 60 foot by 60 foot fenced area with (24) 6 foot high evergreen trees. 5. The applicant shall have one (1) year from the issuance of this land use permit to complete construction and install landscaping. If the permit expires, the applicant will be required to secure a new land use permit. 6. The applicant shall notify the Zoning Administrator of any change in name or ownership of the approved facilities and provide new contact information for the purpose of keeping the file updated. 7. Any minor change (or addition) in the project shall require prior review and approval by the Zoning Administrator. Any major change and/or addition to the originally approved plans will require the submittal of a new land use permit application. 8. Within 12 months of cessation of operation or use of the wireless facilities, all unused antennas and equipment shall be removed at the applicant's expense. After the facilities are removed, the site shall be restored to its original condition or an improved condition. This approval does not allow for any structures (temporary or permanent), activities, or development beyond the limits of this request. Your information will remain on file in the St. Croix County Planning and Zoning Department. Please contact the Town of Richmond to obtain the necessary building permits for the proposed construction.
