STATE /COUNTY CONTRACTS AND ADDENDA

WHEREAS, the State of Wisconsin, Department of Health and
Social Services (DHSS) allocates monies to the St. Croix County
Department of Human Services through State /County contracts and
addenda; and
WHEREAS, DHSS requires that these contracts and addenda be
executed in a timely manner by the County Board Chair or designee
as appointed by the County Board; and
WHEREAS, the Administration of State /County contracts and
all contract addenda would be more efficient through the
appointment of the Human Services Board Chair as Co- designee for
the County Board Chair; and
NOW THEREFORE, BE IT RESOLVED by the St. Croix County Board
of Supervisors, meeting in regular session, that the following
persons are hereby authorized to execute all contracts for the
St. Croix County Department of Human Services:
1. County Board Chairman,
2. County Board Vice - Chair, or
3. Human Services Board Chair
BE IT FURTHER RESOLVED, that where time permits, the
District Attorney, or his designee, shall review all such
contracts prior to execution, per insurance procedure presently
in existence in St. Croix County.
