WHEREAS, as a result of the closing of the Tannenbaum Nur-
sing Home in Burkhardt, Wisconsin on April 4, 1986, St. Croix
County became eligible to participate in the Community Integra-
tion Program II (CIP II); and
WHEREAS, the Community Integration Program II is a federal
program administered by the Wisconsin Department of Health and
Social Services, Division of Community Services, that allows the
diversion of Medical Assistance dollars for nursing home care
costs to community living arrangement costs; and
WHEREAS, the Social Services, Health and Unified Services
Committees of St. Croix County recommends the submission of a
letter of intent to the Office of Program Initiatives, Division
of Community Services, notifying them of the County's intent to
participate in the program, effective September 1, 1986; and
WHEREAS, CIP II guidelines require that the County Board of
Supervisors designate a lead agency in the County to administer
the said program,
NOW THEREFORE, BE IT RESOLVED that the St. Croix County
Board of Supervisors, meeting in regular session, hereby approves
St. Croix County's participation in the Community Integration
Program II, as well as the submission of a letter of intent to
participate in said program, and
BE IT FURTHER RESOLVED that the St. Croix County Board
designates the Department of Human Services as its lead agency to
administer said program.
