The following conditions shall be met during construction or installation and prior to occupancy or use: Key Item(s) • In the event this soil absorption system or any of its component parts malfunctions so as to create a health hazard, the property owner must follow the contingency plan as described in the approved plans. In addition, the owner must insure that the operation, maintenance and monitoring duties as described in section VIII of the mound component manual are complied with. A copy of this information must be given to the owner upon completion of the project. • The designer proposes to install a state approved effluent filter to achieve the requirement of wastewater particle size. Pursuant to outlet filter product approval stipulations, maintenance information must be given to the owner of the POWTS explaining that periodic cleaning of the effluent filter is required. The access opening used to service the filter shall terminate at or above finished grade with a watertight cover. • The gravelless system components must be installed in accordance with the manufacturer's printed instructions, the plan approval, and COMM 83 system sizing criteria. If there is a conflict between the manufacturer's instructions and tlieplan approval, the ' 1 take precedence. Reminder The orientation of the mound system must be such that the longest dimension is oriented along the ace contour per COMM 83.44(6)(x)2. Limit activities in the area 15' beyond the down slope edge of the mound per Mound Component Manual. A copy of the approved plans, specifications and this letter shall be on-site during construction and open to inspection by authorized representatives of the Department, which may include local inspectors. All permits required by the state or the local municipality shall be obtained prior to commencement of construction/installation/operation. In granting this approval the Division of Safety & Buildings reserves the right to require changes or additions should conditions arise making them necessary for code compliance. Tank inspections must include a visual inspection of the tank(s) to identify any missing or broken hardware, identify any cracks or leaks, measure the volume of combined sludge and scum and to check for any back up or ponding of effluent on the ground surface. The dispersal cell(s) shall be visually inspected to check the effluent levels in the observation pipes and to check for any ponding of effluent on the ground surface. The ponding of effluent on the ground surface may indicate a failing condition and requires the immediate notification of the local regulatory authority. When the combined accumulation of sludge and scum in any tank equals one-third IY3) or more of the tank volume, the entire contents of the tank shall be removed by a Septage Servicing Operator and disposed of in accordance with chapter NR 113, Wisconsin Administrative Code. All other services, including but not limited to the servicing of effluent filters, mechanical or pressurized components, pretreatment units, and any servicing at intervals of 512 months, shall be performed by a certified POWTS Maintainer. A service report shall be provided to the local regulatory authority within 10 days of completion of any service event. Do not drive or park vehicles over tanks and dispersal cells. Do not drive or park over, or otherwise disturb or compact, the area within 15 feet down slope of any mound or at-grade soil absorption area. Reduction or elimination of the following from the wastewater stream may improve the performance and prolong the life of the POWTS: antibiotics; baby wipes; cigarette butts; condoms; cotton swabs; degreasers; dental floss; diapers; disinfectants; fat; foundation drain isump pump) water; fruit and vegetable peelings; gasoline; grease; herbicides; meat scraps; medications; oil; painting products; pesticides; sanitary napkins; tampons; and water softener brine. ABANDONMENT When the POWTS fails and/or is permanently taken out of service the following steps shall be taken to insure that the system is properly and safely abandoned in compliance with chapter Comm 83.33, Wisconsin Administrative Code: • All piping to tanks and pits shall be disconnected and the abandoned pipe openings sealed. • The contents of all tanks and pits shall be removed and properly disposed of by a Septage Servicing Operator. • After pumping, all tanks and pits shall be excavated and removed or their covers removed and the void space filled with soil, gravel or another inert solid material. CONTINGENCY PLAN If the POWTS fails and cannot be repaired the following measures have been, or-must be .taken, to provide a code compliant replacement system: ^ A suitable replacement area has been evaluated and may be utilized for the location of a replacement soil absorption system. The replacement area should be protected from disturbance and compaction and should not be infringed upon by required setbacks from existing and proposed structure, lot lines and wells. Failure to protect the replacement area will result in the need for a new soil and site evaluation to establish a suitable replacement area. An additional hand boring was completed by CST Mary Jo Hollister to enlarge the original soil tested area. The revised plot plan provided corrections in the locations for borings 2SA and 2SB, the direction of slope, and established new benchmarks for determining boring elevations. The weather was sunny, temperature in the mid- 70's, with mixed sunshade from tree cover and no precipitation for >24 hours.. Test pits 2SA and 2SB were discernible from backfill material and the hand boring done in August was located north of pit 2SA. None of the locations are marked, but were confirmed based on distances from southwest property corner and road right- of-way. (I measured and noted the distances on a copy of the plot plan) On Sept. 14`" Mark Iverson, Cedar Corp. Professional Soil Scientist, accompanied me to double- check the grade and structure of Horizon 2 to make sure the appropriate soil application rate was being described and recommended for use in POWTS design. My hand boring was located adjacent to pit 2SB, measured 90 ft. east of BM2 and 12 ft. north of the southerly property line. Horizon 1: 0 - 7" 10 YR 2/2 silt loam, moderate (2) fine-medium granular peds with Moist, friable consistence, many roots ranging fine to coarse. Horizon 2: 7 - 14" 10 YR 4/4 - 4/3 silt loam, moderate (2) fine subangular blocky peds with dry/soft consistence (due to dry conditions, the excavation difficulty was moderate to high) Some 10 YR S/3 silt loam was mixed in, but color variations did not appear to be a redoximorphic feature. Mark Iverson agreed that this didn't appear to be mottling. ,.,, - ~""~~_ Another hand boring was excavated approximately 20 feet east of the above described boring and conditions observed were essentially the same. Redox features were reported in the hand boring at 17 inches, so the recommended 1.59 feet of sand fill and soil application rate of 0.6 gpd/ft2 appears to be appropriate for a mound design on this site. Based on the recommended rate, a 4-bedroom, 600 GPD design flow would require a minimum basal area of 1000 sq. ft. The test pits and hand borings create limitations for mound system design based on proximity to the south property line and having only 60-65 feet along a 98.5' contour line. The tested area is wooded and careful site preparation (tree and underbrush removal) will be necessary prior to system installation to prevent damage to surface soils. The basal area plowing must be with a chisel-type implement, not a frost tooth, to break up Horizon 2 and allow sand to mix into the subsoil. We recommend the mound site be staked and marked to prevent construction equipment or other traffic accessing the lot from the southwest corner. Dear Ms. Hollister: Our office recently received a request to examine the Soil Evaluation Reports for Forest Ridge Estates with regard to potential POWTS design problems. The following comments summarize reviewer findings and additional information required before these reports can be used to design code-compliant POWTS: 1. The Mound Component Manual specifically restricts Linear Loading Rates to 4.5 gal/ft./day or less for any system with in situ soils having <0.3 gal/ftz/day within 12 inches of the fill. This means that for a four (4) bedroom, 600 GPD design flow system, the mound distribution cell must be a a~ minimum of 133.33' long. A 3 bedroom, 450 GPD system would require at least 100' in length. --~ ,rjy The following lots have soil application loading rates of 0.2 gpd/ftz, but do not have tested areas ,,R~''" that allow for a long, narrow mound: 4 (if system is placed near boring P-24), 5, 6, 11, 13, 14, (`~'~ 17, 18, 28, and 30. 2. Lots 22, 25 26, and 27 have a >0.3 gpd/ft~ loading rate, but due to driveway locations and property ine setbacks it may not be possible to design a mound until the tested area is extended or enlarged. As previously requested, all mound areas are to be staked and preserved within ~N ~%~,.,~ this subdivision due to the number of borings encountered that have soil conditions unacceptable ~"`"~ for mound design (less than A+4'~. 3. Several soil reports have boring elevations that appear inaccurate when compared to the corresponding locations shown on the sit{;plus. The soil tester must verify and/or correct 13~ elevations reported on the following lots. 7, O~benchmark elevations were provided), 15, 2I 16, 18, and 20 & 21 P-49 had an elevation of 1198.24' on Lot 21 and 1197.61' on Lot 22). ~~ When using sea level elevations, the same ormg s ould remain unchanged, even if different ~ ~ benchmarks are used, because it doesn't move on the landscape. [On lots 13 and 14, the borings 14-A and P-69 did change elevations on each report because different benchmarks with an assumed 100.0' elevation were used.] 4 Correction is needed for notations where Structure is reported using, for example, "if-L sbk". ~~~~~oy The field book listing for size doesn't include an "L", but uses CO to indicate large or coarse Hollister ,Page 2 structural units. Although it can be interpreted to mean ~~fine grading to large" peds, soil reports must stick to accepted soil terminology. Due to current frozen, snow-covered conditions, we realize the additional soil information will not be immediately available. However, plans should be made to schedule the site work so that these reports will provide enough data for POWTS designs to be developed and sent to the State for plan review. Sanitary permits cannot be issued for the lots referenced above until the soil reports in question have been corrected and/or completed.
