RESOLUTION DESIGNATING THE HEALTH AND HUMAN
SERVICES DEPARTMENT AS THE ADMINISTERING
_ AGENCY FOR THE COORDINATED SERVICES TEAM
INITIATIVE
1 WHEREAS, the St. Croix County Department of Health and Human Services has
2 established a Coordinated Services Team (CST) Initiative to deliver support and services for
3 families who are involved in multiple systems; and
4
5 WHEREAS, the State Legislature appropriated additional General Purpose Revenue to
6 expand CST Initiative for State Fiscal Years 2014 and 2015; and
7
8 WHEREAS, the Department of Health and Human Services is in the process of
9 submitting an application to receive an amount of approximately $62,123 in funding; and
10
11 WHEREAS, pursuant to Wisconsin Statute §46.56(2) the County Board shall designate
12 an administering agency for the CST Initiative.

14 THEREFORE be it resolved by the St. Croix County Board of Supervisors that the
15 Health and Human Services Department be designated as the administering agency for the CST
16 Initiative.

Legal Note: None
Fiscal Impact: This resolution will make St. Croix County eligible for $62,123 in state funding.
County funding requirement of 20% cash or in-kind match is in the 2014 budget and
does not require additional tax levy.
