7. The Wisconsin Department of Natural Resources has reviewed the plans and recommends that storm water runoff from the hard surfaces on the site be managed. Based on these findings, approval of the land use permit is subject to the following conditions: 1. Prior to commencing construction, the applicant shall be responsible for properly installing silt fencing or some other type of sediment control measure around the perimeter of the construction site and down gradient from the proposed garage. The applicant shall maintain all erosion control measures until permanent, self-sustaining vegetation is successfully established on all disturbed areas of the site. 2. The applicant shall contact the Zoning Administrator when all construction is completed. 3. No phosphorous fertilizers shall be used to establish or maintain a lawn on the disturbed areas of the site or elsewhere on the property,unless a soil test confirms that phosphorous is needed. 4. Mowing shall be prohibited within the wetland boundary, unless prior approval is obtained from the Zoning Administrator to eradicate invasive species. The applicant shall be responsible for immediately restoring and maintaining native vegetation within the wetland. 5. Within 30 days of this approval, the applicant shall plant additional native trees, shrubs, and groundcover along the shoreline of Squaw Lake and within 35 feet landward of the Ordinary High Water Mark (OHWM)in order to: • substantially screen the garage from the lake during summer leaf-on conditions. • filter and slow storm water runoff from the site before reaching the lake, • prevent erosion and sedimentation along the shoreline,and • preserve the natural ecology and wildlife habitat along the shoreline. 6. Within 30 days of completing construction,the applicant shall execute and record an affidavit against the property referencing a long-term maintenance agreement for the native vegetation within the wetland boundary (including a provision prohibiting mowing)and along the shoreline of Squaw Lake and within 35 feet landward of the OHWM with the Register of Deeds,and provide the Zoning Administrator with recorded copies of these documents.The purpose of this is to alert future property owners of the responsibilities and limitations in maintaining those areas. 7. Within 60 days of completing construction,the applicant shall submit to the Zoning Administrator photos of the completed project as viewed from all angles,including from the shoreline of Squaw Lake. The photographs must also show the wetland and shoreline vegetation on the site. This approval does not allow for any additional construction, structures or structural changes, grading, filling, or clearing of vegetation beyond the limits of this request. Your information will remain on file in the St. Croix County Planning and Zoning Department. It is your responsibility to ensure compliance with any other local, state, or federal rules or regulations, including but not limited to obtaining a building permit from the Town of Star Prairie.
