Owner Responsibilities: • SPS 383.52 Responsibilities. The owner of a POWTS shall be responsible for ensuring that the operation and maintenance of the POWTS occurs in accordance with this chapter and the approved management plan under s. SPS 383.54(1). • SPS 383.52(2) A POWTS that is not maintained in accordance with the approved management plan or as required under s. SPS 383.54(4) shall be considered a human health hazard. • SPS 383.55 The owner is responsible for submitting a maintenance verification report acceptable to the county for maintenance tracking purposes. Reports shall be submitted at intervals appropriate for the component(s) utilized in the POWTS. All permits required by the state or the local municipality shall be obtained prior to commencement of construction/installation/operation. Existing vegetation is mowed and raked off the site. The mound basal area (L x W) is plowed with a moldboard or chisel plow. Plowing may not proceed if the soil is wet enough at the plow depth to form a'/4 inch soil wire when a sample is rolled between the palms of the hands. ASTM C-33 quality sand is placed immediately after plowing. Sand is placed with a tracked machine keeping 12 or more inches of sand under the tracks or is placed overhead by a backhoe. Special care must be used when placing sand of less than one foot thickness to minimize compaction of the plowed surface. After the topsoil cap is placed, the entire mound is seeded and mulched to promote vegetative growth, limit erosion and protect from freezing. While holding the cartridge on its side (large flat surface facing ; down; over the access opening, rinse off the cartridge with water only, making sure all septage material is rinsed back into the tank. 8. If VRS switch is utilized, replace by inserting into filter and turning clockwise 90 "91 9. The following summary provides specific data that will be necessary before these reports can Real operty _ be used to design mound private on-site wastewater treatment systems 7 -386-4677 (POWTS): G7'l(G~ / "ljp R cling -386-4675 Lot 17 - Due to extensive grading at the rear of this lot, adjacent td a drainage easement and walking trail, the NE corner iron pipe benchmark may have been altered and its elevation may not be accurate for POWTS installation. You didl not correct the problem with alternate Benchmark at elevation 1075.24', which is shown on the plot plan as being located higher than the 1076' contour line. Boring B17B elevation is at 1076.28' and appears to be below the benchmark onVJ the plot plan. (Note: This same benchmark is also wrong on lot 18 soil report, but another benchmark can be used for that system's design) In addition, you cannot arbitrarily modify the contours for the site plan from the original engineer's plan. You need to find the benchmark and obtain an accurately elevation so that a POWTS installer can use it to establish a contour for the mound. Lot 22- You cannot add contour lines to the engineer's drawing to make the contour elevations work. The corner iron pipe elevations and borings B-22A and B-22C appear correct, but B-22B had an elevation of 1033.27' at a location on the plot plan between contours 1026' and 1028'. Either the boring was much higher on the slope than indicated on the plot plan or a math error was made when Ogden's survey crew gave you the elevation data. By modifying the plot plan to add contours, you now created a 22% slope where a 7% slope was originally shown. Has the site been cut or altered?? You need to verify the actual elevation on your boring B-22B and provide that data before a mound design can be developed for this lot. ST.
