The submittal described above has been reviewed for conformance with applicable Wisconsin Administrative Codes
and Wisconsin Statutes. The submittal has been CONDITIONALLY APPROVED. The owner, as defined in
chapter 101.01(10), Wisconsin Statutes, is responsible for compliance with all code requirements.
No person may engage in or work at plumbing in the state unless licensed to do so by the Department per
s.145.06, stats.
The following conditions shall be met during construction or installation and prior to occupancy or use:
• This system is to be constructed and located in accordance with the enclosed approved plans and with the "In-
ground Soil Absorption Component Manual for Private Onsite Wastewater Treatment Systems VERSION 2.0"
SBD-10705-P (N.O1/O1).
• This system is to be constructed and located in accordance with the enclosed approved plans and with the
"Holding Tank Component Manual for Private Onsite Wastewater Systems" SBD-10571-P (R.6/99).
A copy of the approved plans, specifications and this letter shall be on-site during construction and open to
inspection by authorized representatives of the Department, which may include local inspectors. All permits
required by the state or the local municipality shall be obtained prior to commencement of FT.

Sizing Calculation for the Hudson Rod, Gun and Archery Ctub
The sizing for this system will be based upon the dining hall public wastewater flow of 7 gallons
per meal served. The rationale for this choice is that only a single meal will be served on those
days that meals are prepared. It is my understanding that most of the meals will be prepared off
site and catered in.
The club members have chosen to split the wastewater flow from the kitchen and restrooms to
eliminate the potential of high strength waste, which would require pretreatment. The kitchen
waste will be served by a holding tank and the bathroom wastes by a conventional lift system.
Kitchen waste = 690 ~aVdav estimated.
160 meals x 4 gallon = 640 gal.
2 Floor drains x 25 gallons = 50_gal.
Total estimated daily flow = 690 gal. 690 gal, x 1.5 design factor = 1035 DWF
Holding tank sizing = 690ga1./day x 5 day estimated daily wastewater flow = 3450 gal.
Plus the grease interceptor requirement of 1440 gal. = 4890 gal. total required holding tank

Septic Tank And Gravity fn-Ground Soil Absorption Component
Pursuant to Comm 83.54 Wis. Adm. Code each Private Onsite Wastewater Treatment
System (POWYS) shall include information and procedures for maintaining the system within
the parameters of Comm 83 and 84, and the conditions of approval by the department, agent,
or governmental unit. The approved plans and permits for system are on file at the county
zoning or health department.
This management plan complies with Comm 83.54, Wis. Adm. Code, and the !n-Ground
Soil Absorption Component Manual for Private Onsite Wastewater Treatment Systems SBD-

Seutic Tank
The septic tank shall be maintained by an individual certified to service septic tanks
under s. 281.48, Stats. The contents of the septic tank shall be disposed of in accordance with
NR 113, Wis, Adm. Code (Servicing Septic or Holding Tanks, Pumping Chambers, Grease

Interceptors, Seepage Beds, Seepage Pits, Seepage Trenches, Privies, or Portable
Restrooms).
The operating condition of the septic tank and outlet filter shall be assessed at (east
once every 3 years by inspection. The outlet filter shall be cleaned as necessary to ensure
proper operation. The filter cartridge should not be removed unless provisions are made to
retain solids in the tank that may slough off the fitter when removed from its enclosure. If the
filter is equipped with an alarm, the filter shall be serviced if the alarm is activated continuously.
Intermittent fitter alarms may indicate surge flows or an impending continuous alarm. The septic
tank sha{t have its contents removed when the volume of scum and sludge in the tank exceeds
113 the liquid volume of the tank. If the contents of the tank are not removed at the time of an
assessment, maintenance personnel shall advise the owner of when the next service needs to
be performed to maintain less than maximum scum and sludge accumulation in the tank.
Pump Chamber
A pump chamber if part of the component package also requires monitoring once every three
years or at the time of a problem, complaint or failure. Inspection should include checking the
dose rate, volume and frequency.
Warning: The dose chamber may fill due to flow continuing during pump
malfunction or power outages. One large dose when the power comes on
or when the pump is repaired may cause the dispersal system to have
problems. In this situation, the pump chamber should be pumped by a
licensed pumper before pump cycling begins or other measures shall be
used to dose the component with only the proper amount of influent. This
may include manual operation of the pump controls until such time the
pump chamber has reached its normal level.
Septic tank and Pump chamber manhole risers, access risers and covers should be
inspected for water tightness and soundness. Access openings used for service and
assessment shall be sealed watertight upon the completion of service. Any opening deemed
unsound, defective, or subject to failure must be replaced. Exposed access openings greater
than 8-inches in diameter shall be secured by an effective locking device to prevent acadenta!
or unauthorized entry into the tank.
No one should enter a septic or other treatment or holding tank for
any reason without being in full complianc® with OSHA standards for
entering a confined space. The atmosphere within the septic or other
treatment or holding tank may contain lethal gases, and rescue of a
person from the interior of the tank maybe difficult or impossible.
Tank abandonment shall be in accordance with Comm 83.33, Wis. Adm. Code when the
tank is no longer used as a POWfS component.

Management Plan for a Septic Tank and Soil Absorption Component
The soil absorption component serving this structure is designed to accept commercial
wastewater from a commercial facility. The limits of operation of this component are shown in
Table 2.
The longevity of a soil absorption component depends greatly on proper and timely
maintenance, and system use within or below the limits of reliable operation. Good water
conservation practices by all occupants and the installation of water conserving plumbing
fixtures are key factors in extending the useful life of this component.
The soil absorption component's operation must be assessed by inspection at least
once every three years. The inspection shall include recording the levels of ponding, if any, in
the observation pipes, and a visual inspection for any evidence of surface seepage or
discharge from the component. On steeply sloping sites, areas of erosion should be identified
and reported to the owner for repair. The surface discharge of domestic wastewater or sewage
from the system is prohibited and considered a human health hazard.
Traffic around or over the soil absorption component should be avoided particularly
during winter months. The compaction or removal of snow cover over the component may lead
to hydraulic failure by freezing. This type of failure is usually temporary, but is difficult or
impossible to repair until weather conditions improve. In general, soil compaction over this
component will reduce diffusion of oxygen into the soil and dispersal cell, which may lead to
more intense, and earlier, organic clogging of the soil.
Plantings of deep-rooted trees and shrubs dirnctly over or within ten feet of the
component should be avoided since root intrusion into the component may obstruct wastewater
flow.
The soil absorption component reserve area if one is provided should be kept free of
any structural encroachments and left undisturbed.
Contangency Plan
A contingency plan for this site would be to install a new system in the tested area
available or excavate existing leaching chambers to determine component failure, then
remove the clogging mat and replace with ASTM G33 sand if necessary. Once this is
completed, Gean and replace leaching chambers and backfill. This may require and individual
site design.

1. The owner agrees to file a copy of this contract with the local goven~unental unit that has signed the
pumping agreement required in Comm 83.52(1)(c)1. Wis. Adm. Code and the approved Holding Tank
Component Manual. This agreement will also be filed with the St. Croix County Zoning Department.
2. The owner agrees to have the holding tank(s) serviced by the pumper and guarantees to permit the pumper
to have access and to enter upon the property for the purpose of servicing the holding tank(s). The owner
agrees to maintain the access road or drive so that the pumper can service the holding tank(s) with the
pumping equipment. The owner further agrees to pay the pumper for all charges incurred in servicing the
holding tank(s) as mutually agreed upon by the owner and pumper.
The pumper agrees to submit to the local governmental unit that has signed the pumping agreement and to
the County, a report for the servicing of the holding tank(s) on a semiannual basis. The pumper further
agrees to include the following in the semiannual report:
a. The name and address of the person responsible for servicing the holding tank;

c. The location of the property on which the holding tank is installed;
d. The sanitary permit number issued for the holding tank;
e. The dates on which the holding tank was serviced;
f. The volume in gallons of the contents pumped from the holding tank for each servicing;
g. The disposal sites to which the contents from the holding tank were delivered.
4. This agreement will remain in effect until the owner or pumper terminates this contract. In the event of a
change in this contract, the owner agrees to file a copy of any changes to this service contract or a copy of a

1 . Owner agrees to conform to all appUcable regWrements of Ch. Comm 83, Wls. Adm. Code rotating to hblding tanks. If the ovmer falls to have
the holding tank properly servtoad In response to orders issued by the governmental un{t of the Department of Commerce to prevent or abate
a human health hazard as described in s. 254.68, Stets., the governmental unit (Town) may enter upon tits property and service the tank or
cause to have the tank to bs serviced and charge the owner by ptadnp the dtarges on the. tax bill as a spade/ assessment for ttrrrent
services rendered. The charges w1U bs assessed as preat:ribed by s. 68.0703, 3tats.
2. The owner agrees, pursuant to s. Comm 83.54 (2), and Comm 82.40(3xe), Wis. Adm. Coda, •to-hays a•water meter Installed in ttw structure.
The water mater shall be ktatalled by a plumber authorized by the Department of Commerce to itiakit such instagaUons, with said inetatlatton
compiytng with State regulations and manufacturers spedflcatlons. The dvmer agrees to ~bp flnaadally reaponslWs for-the pun:haw,
Installation, maintenance, and repeU of the water molar, and agrees to allow the governmental unit or the Department of Commerce to enter
the above-described property on a regular basis to road and/or inspect the water m®tar.
3. Owner agrees to pay all changes and costs incurred by the governmental unit Or county for InspecUOn, pumping, hauling, tX otherwise
servidng and maMtatninp the holding tank in such a manner sa to prevent or abate any human health herald caused by the hoding tank.
The governmental unl! shall notify the owner of any costs which shall be'paid by the owner within thirty (30) days from the date of notlce. In
the event the owner does not pay the coats witMn thirty (30) days, the owner spedOcally egress that all the Deets and dtaryes may be placed
on the tax roll ss a spade/ assessment for the abatement of a human health hazard, and the tax shall be collected es provided by taw.
4. Ths owner, agrees to contract with a person who la licensed under Ch. NR 173, Wls. Adm. Code, to have the ttokling tank saMced and to
file a wpy of the contract with the governmental unit, The owner further agrees to file a Dopy of any changes to tits servlq contract, or a
copy of a new ssMco contract, with the govemmsntal uMt within ten (10) bustMSS days from the data of diarige to the seMce centred.
5. The owner agrees to contract with a parson Ucansed under Ch. NR 113, Wia. Adm. Code, who shall submit to the county on s semiannual
bests a report detalUng the servidrtg of ttta holding tank. Tha governmental unit or county may enter upon the properly to Investlgate the
condition of the holding tank when pumping reports and meter readings may indicate that the holding tank is not being properly maintalrted.
8. This agreamont will remain In eHsct only until the county offtce responsible for the ropulaUon of private onsite wastewater trsstment systems
certifies that the property is served by either a munldpsl sewer or a private ortalts wastewater treatment system that complies with Ch. Comm
83, Wis. Adm. Coda. In additlon, this agreement may be cancelled by executing end recording said certltlcatkxt with reference to this
agreamont in such manner which will permit the existence of the certificatlon to be determined by reference to the property.
~. This agreement shall be binding upon the owner, the halts of the owner, and assignees of the owner. Tha owner shall submit this agreement
to the register of deeds, and the agreement shall be recorded by the register of deeds In a manner which wilt psnnit the existence of the
