ST RO .,, RESOLUTION AUTHORIZING THE INITIATING OF THE
' ' SOAR PROGRAM IN ST. CROIX COUNTY USING THE
COMMUNITY DEVELOPMENT BLOCK GRANT
1 WHEREAS, WEST CAP Inc. has received Federal monies under the Community
2 Development Block Grant(CDBG)program, administered by the Wisconsin Department of
3 Administration (DOA) Division of Housing (DOH) for the purpose of the provision of SOAR
4 (SSUSSDI Outreach, Access, and Recovery) services; and
5
6 WHEREAS, SOAR is a program for individuals who are homeless or at risk of being
7 homeless and helps them apply for SSUSSDI; and
8
9 WHEREAS, WEST CAP, Inc. is a private, non-profit corporation, with one or more staff
10 members who have been certified by the Substance Abuse and Mental Health Services
11 Administration of the U.S. Department of Health Services as having successfully completed
12 training in the SOAR model; and
13
14 WHEREAS, it is necessary for the St. Croix County Board to approve initiating the
15 program in the county, as well as approve the usage of SOAR funding; and
16
17 WHEREAS, the St. Croix County Department of Health and Human Services as
18 authorized by the St. Croix County Board of Supervisors through this resolution and WEST
19 CAP, Inc. will work together to provide SOAR services to eligible recipients within a DOH
20 SOAR program service area; and
21
22 WHEREAS, the St. Croix County Health and Human Services Department will work
23 with WEST CAP Inc. on initiating the project within the county; and
24
25 WHEREAS, St. Croix County acknowledges, understands and agrees that WEST CAP,
26 Inc. may use CDBG grant funds received from DOH to provide SOAR services to eligible
27 recipients present in St. Croix County up to the amount of available grant funds.

29 THEREFORE, be it resolved, that the St. Croix County Board of Supervisors does hereby
30 approve and authorize initiating SOAR program project.
