Arrangements for inspection shall be made with the designated county official in accordance with the provisions of Sec. 145.20(2)(d), Wis. Stats. • A state approved effluent filter is required. Maintenance information must be given to the owner of the tank explaining that periodic cleaning of the filter is required. Access to the filter for cleaning must be provided per SPS 384 product approval conditions. • All POWTS component piping material shall be SPS 384, Wis. Adm. Code compliant. • The area within 15' downslope of the dispersal cell shall remain undisturbed. Vehicular traffic, excavation or soil compaction is prohibited in this area. • A copy of the approved plans specifications and this letter shall be on-site during construction and open to inspection by authorized representatives of the Department which may include local inspectors BENNIE W HELGESON Page 2 8/10/2016 Owner Responsibilities • The current owner, and each subsequent owner, shall receive a copy of this letter including instructions relating to proper use and maintenance of the system. Owners shall receive a copy of the appropriate operation and maintenance manual and/or owner's manual for the POWTS described in this approval. • The owner of a POWTS shall be responsible for ensuring that the operation and maintenance of the POWTS occurs in accordance with this chapter and the approved management plan under s. SPS 383.54(1). • In the event this soil absorption system or any of its component parts malfunctions so as to create a health hazard, the property owner must follow the contingency plan as described in the approved plans. • The owner is responsible for submitting a maintenance verification report acceptable to the county for maintenance tracking purposes. Reports shall be submitted at intervals appropriate for the component(s) utilized in the POWTS. In granting this approval the Division of Industry Services reserves the right to require changes or additions should conditions arise making them necessary for code compliance. As per state stats 101.12(2), nothing in this review shall relieve the designer of the responsibility for designing a safe building, structure, or component. Tank inspections must include a visual inspection of the tank(s) to identify any missing or broken hardware, identify any cracks or leaks, measure the volume of combined sludge and scum and a check for any back up or ponding of effluent on the ground surface. The soil absorption system shall be visually inspected to check the effluent levels in the observation pipes and to check for any ponding of effluent on the ground surface. The ponding of effluent on, the ground surface may indicate a failing condition and requires the immediate notification of the local regulatory authority. When the combined accumulation of sludge and scum in any treatment tank equals one-third ('f3) or more of the tank volume, the entire contents of the tank shall be removed by a Septage Servicing Operator (pumper) and disposed of in accordance with chapter NR 113, Wisconsin Administrative Code: All other services, including but not limited. to the servicing of effluent filters, mechanical or pressurized components, pretreatment units, and any servicing at intervals of 512 months, shall be performed by a certified POWTS Maintainer. A service report shall be provided to the local regulatory authority within 30 days of completion of any service event. The site has a minmum of 6" of unsaturated in situ soil for treatment and dispersal of treated wastewater effluent and is therefore suitable for a mound POWTS as allowed by SPS 383.44(3)(b)1. Recommendations: 1.) The mound dispersal cell should be constricted on the 88.75' contour in a linear shape as indicated on the Soil & Site Evaluation Report. The dispersal cell should be designed and installed using as much of the available contour length as possible - approximately 100.0'. 2.) Thirty inches (30") of ASTM-C33 sand fill shall be placed beneath the upper edge of the dispersal cell. 3.) The linear loading rate for application of septic tank effluent should be 4.5 gpd/linear ft. or less and the basal area application rate should not exceed 0.20 gpd'sq. ft. The downslope toe of the mound must be extended to provide additional basal area. 4.) The mound shall be constructed to divert surface water around and away from the mound. A surface water diversion trench should be constricted at approximately 10' wide extending between 10' - 20' upslope of the mound site. Additional fill should also be added directly upslope of the upper toe of the mound and tapered into the diversion trench and graded to direct surface water around and away from the mound area. 5.) The mound shall be constricted in accordance with Dep't. of Commerce regulations and recommendations including the removal of all vegetation from the site prior to site preparation. 6.) The site should be chisel plowed to a minimum depth of 12". A+O" Interpretive Determination Report Mellgren Property Introduction A Soil & Site Evaluation of this site was completed by James K. Thompson, Dep't. of Safety & Professional Services Credential #30021 on April 270', 2015 . The purpose to determine the suitability of the site to accommodate a replacement Onsite Wastewater Treatment System (POWTS) to serve an existing residence located on the property. The existing parcel contains approximately 40 acres, primarily consisting of productive and fallow agricultural land. The results of the evaluation indicated that soil suitability is uniform throughout the site, with suitable depths of soil ranging from 7" - 9" (A+0"). An onsite soils verification was completed by Ed Taylor, Dep't. of Safety & Professional Services Wastewater Specialist, on April 271h 2015. Ryan Yarrington, St. Croix County Zoning Specialist and Chris Lickness, property owner. were present to assist. Project information: The proposed mound site is located in the northwest corner of the NW 1/4NW 1/4t Sec. 33. T.28N. R. 16W., Tn. of Eau Galle, St. Croix County. WL, lying approximately 625' south of the center line of 10"' Ave. and 250' east of centerline of 25W' St. The site has been untended pasture for several years. Methods Soil color, texture, consistence, and Redoximorphic features have been evaluated and recorded on the attached Soil & Site Evaluation Report in accordance with Chapter SPS 385.60(2), Wisc. Administrative Code. The initial field investigation of the site included evaluation of 6 hand dug pits, two of which were verified by Mr. Taylor. One-foot contour intervals were determined within and surrounding the proposed system location by collecting rod and transit elevations at all soil pits and at 20' grid intervals throughout the proposed system area. Additional elevations and horizontal measurements were taken as needed to identify landscape position and determine separation distances from dominant landscape forms. Local Geology St. Croix County is underlain by Prairie du Chien limestone and St. Peter sandstone. which is overlain with glacial till, fornung the principal soil parent material. A search of Department of Natural Resources Well Drilling Reports filed for wells constructed within the geographic region indicated soil depths of 104'- 138' overlying Dolomitic limestone. Static water levels are found at 150'- 205' below grade. Geomorphology Parent Material: The parent material of this site is consistent with the mapping units - Freeon silt loam - as shown in the U.S.G.S. Soil Survey Manual, consisting of sandy loam glacial till overlain by a post glaciation Aeolian loess cap. These soils are moderately well drained with moderate permeability. Landscape position: The tested area is positioned on the back slope of a glacial ground moraine at a nominal Mean Sea Level (MSL) elevation of 1212'. The proposed system contour lies approximately 10' below a lmiform, convex summit slope approximately 200' west of the site, with the moraine extending gradually upslope to the north - northwest to a nominal elevation of 1230' MSL. The ground moraine extends downslope from the tested area in a south - southeasterly direction approximately 170' to an unnamed intermittent drainage way at the toe slope having a nominal elevation of 1207' MSL, continuing southerly to an eventual elevation of 1204' MSL. Slope and Aspect: The contour within the proposed system area is smooth linear, with a slope shape that is linear along the horizontal contour and linear along the vertical contour. The ground slope is consistent at 2'/%) - ,'i6 with a southeast aspect of 122°. 1 Local Hydrolo;,v: Surface Water Drainage: Surface water drainage is uniform throughout the proposed system footprint and the surrounding area witlun 100'. The smooth linear slope throughout the tested. area disperses surface water flow away from the proposed system area. There are two areas of concentrated flow. The first being the intermittent water way discussed above, which is located approximately, 170' east of the tested area and drains in a southerly direction. A second intermittent water way is located approx 105' south of the tested area and drains in an easterly direction. The two drainage ways converge approximately 175' south east of the tested area with the water flow continuing on in an easterly direction where it eventually exits the property 1,200' to the east and continues on for an undetermined distance. No artificial means of surface and/or subsurface drainage or ponding were found within 100' of the proposed system area. Subsurface Water Drainage: The A and B horizons consist of moderately structured silt loam providing good infiltration potential. When combined with the uniform 21/% - 3% slope across the proposed system area, this site should provide good lateral subsurface water flow. Soil Saturation: Soil pits were evaluated throughout this property on several occasions throughout the months of March & April. 2015. Soil pits were evaluated to depths ranging from 22" - 63". Saturated soil conditions were encountered at depths ranging from 36" - % 63". Field Observations: Soil Morphology The soils within the proposed system area Nvere found to be consistent with the Frecon mapping unit as shown in the U.S.G.S. Soil Survev of St. Croix County. Wisconsin. Moderately structured sill loams were observed ranging to a depth of 19" -26" withweakly structured silt loams below ranging from 19" - 28". Underlying the silt loams are weakly structured sandy loams and sandy clay loams at depths of 25" - 34". Redoximorphic Features: Redoximorphic features were not observed within the A horizon of any of the soil profiles, with all profiles displaying redoximorphic features at A+0". No evidence of prolonged saturation was observed within the upper 6" of the A horizon. Redoximorphic concentrations were observed as Fe concentrations within the upper portion of the B horizon and throughout the C horizon. The lack of hydric soils indicate that the site is likely saturated for less than 5% of the growing season. This, coupled tiyith the lack of redoximorphic depletions in the upper horizons and the strong matrix colors displayed throughout the soil profiles, indicate limited saturation within these soils. The redoximorphic features observed are likely a result of short-term seasonal saturation. The moderately structured silt loam soils in the A horizon and upper part of the B horizon will accommodate the infiltration and dispersal of applied wastewater from the POWTS and will allow treated effluent to be assimilated into the subsurface soils. The linear slope will accommodate uniform dispersal of both naturally occurring surface water and applied wastewater away from the site. A sodded barnyard area immediately approximately 200 ft east of a barn and approximately 100 ft south of a machine shed was determined to meet the required criteria for state approval of a mound site contingent upon the submittal of an Interpretive Determination Report by the Certified Tester. Soils in the area under consideration formed in a thin mantle of silty loess overlying dense glacial till. The site is positioned on a linear backslope with a three to four percent gradient. The site is located in a Freeon silt loam map unit near its downslope boundary with a Skyberg silt loam map unit of the USDA Cooperative Soil Survey. The Freeon series is moderately-well drained with an expected seasonally saturated zone occurring at 2.0 to 3.5 ft. The Skyberg series is poorly drained with seasonal saturation expected to occur at the surface. The moisture regime of soil observed in two sample profiles on the proposed mound site is somewhat poorly drained with low-chroma Fe-oxide depletions and high-chroma Fe-oxide accumulations present within a l OYR 5/4-3 subsoil matrix beneath a l OYR 3/2 silt loam plow layer that is seven to eight inches thick. No evidence of prolonged saturation was observed in the upper six inches of the plow layer. Structure is moderate granular to moderate subangular blocky throughout the upper twenty inches. To summarize, the site under review is suitable for a mound system contingent upon eventual state approval of an "Interpretive Determination Report."
