The Land use Permit was successfully amended to capsulated the changes done onsite ouring construction. In order to meet the land use permit requirements, you hired engineer Warren White to design ano submit erosion control plans and stormw•ater management design plans. Tnis spring: an onsite was conducted with Warren While for a preconstruction meeting to very how the plans were to be implemented. The onsite conducted on September 21 s12018 was to verify vegetation establishment and to inspect the need for further erosion control measures for ensuring slope stability. The following kt is the findings from the inspection: 1. No erosion control measures were in place. 2. No vegetation growth was established. 3. No storrnw•ater management designs were in place, sediment filled trip- graded retention basin. 4. Gullies have formed along the steep slopes and sediment has washed away. Once construction is complete and vegetation is established, the property owner shall execute and record an affidavit describing the approved storm water management plan and Nicole Hays Ni(ole.Hays(d)sccwi.gov (715)386-4742 maintenance requirements with the County Register of Deeds within 30 days after construction is completed. The affidavit shall alert subsequent purchasers of the land of the maintenance requirements of tie plans. The property owner shall submit a copy of the affidavit a-ong with a record drawing and photos of the storm water retention/detention devices and drainage ways to the Zorii!iq Adminiso'ator. You have been given 30 days to remedy the above findings and to establish vegetation growth. The deadline for this is October 22nd 2018. An onsite will be conducted on this day to verify the findings nave beer, amended. Failure to comply with the above orders will result in this matter being referred to the St. Croix Country Office of Corporation Council for legal action in which a citatior wail be issued. The request for a Land Use Permit is approved based on the application conditions and the following findings: • All setbacks have been met and the Principal Structure meets the 35-foot maximum height requirement. • There will be more than 2,000 sq. ft. of land disturbance on slopes less than 12% within the Shoreland Overlay District. Impervious surfaces currently existing and proposed are approximately 5.5%, and no mitigation is required per WI Admin. Code NR 115 unless proposed impervious surface exceeds 15%. • Erosion and sediment control plans have been provided that meet or exceed WDNR Technical Standards. • The previous principal dwelling has been removed and the foundation is in the process of being removed, then immediate site remediation. Based on these findings, approval of the Land Use Permit is subject to the following conditions: 1. A post-construction inspection will be required prior to removing erosion control measures to determine the entire project site has reached a point of 70% permanent vegetative cover. It is the applicant/agent's responsibility to schedule this with the CDD staff. 2. Erosion control and seeding shall be installed according to the General Erosion Control Notes on the site plan prior to any land disturbance activities. St. Croix County reserves the right to require additional erosion control measures to be installed during construction if found necessary due to site-specific conditions. 3. Failure to comply with the terms or conditions above may result in the revocation of this permit by the Zoning Administrator. C R O I►X C NTY Planning & Land Information Resource Management Community Development Department This permit is valid for one year, with the possibility of up to two (2) six month extensions if the applicant submits the appropriate permit extension fee and documentation to the CDD staff. The permit card is recommended to be posted on the job-site. IT IS THE APPLICANTS/AGENTS RESPONSIBILITY TO ARRANGE INSPECTIONS. FAILURE TO DO SO MAY RESULT IN A CITATION. Prior to an inspection, the CDD staff shall be given at minimum 2 full business days' notice to ensure an inspection time can be arranged. This approval is subject to the conditions listed above; it does not allow flog any add.ivnai clon;t.uctior. structures, grading, paving, filling or clearing of vegetation beyond the limits of this request. The roof is only a 5/12 pitch, so at its highest point it's about 9 feet above the walls. That brings the total height above ground to less than 20 feet maximum. We haven't had the septic installer apply for the septic permit yet. We want to finalize everything with the bank before we do that. We will apply for the septic permit before we apply for our building permit and we will not occupy anything until we're allowed. As soon as the old house foundation is removed, I'll correct the soil in the area for proper drainage and apply grass seed immediately. The total Impervious surface area of all structures existing and proposed, and the entire driveway is 11942 square feet. 11942 divided by 216,896 equals .055..055 times 100 equals 5.5% of impervious surface area. Just to re-iterate what we discussed on the phone, the pond located on the southern portion of the site appears to be Navigable, the Shoreland Overlay District then applies. Based on the map provided the location of the home and garage appears to lie within 300' of the OHWM of the pond. Which means the total Impervious Surface will need to be calculated. The details of this ordinance can be found in Chapter 17.30(J)(2) (Shown below) 2. IMPERVIOUS SURFACE CALCULATION a. The percentage of impervious surface shall be calculated by dividing the surface area of existing and proposed impervious surfaces on the portion of a lot or parcel that is within 300 feet of the Ordinary High Water Mark (OHWM) by the total area of that portion of the lot or parcel that is within 300 feet of the OHWM and multiplying the result by 100. b. Impervious calculations shall not include any area within the Right-of-Way (ROW) or below the OHWM. 2 ! • Then the calculation must meet the requirements of new impervious surfaces. 3. NEW IMPERVIOUS SURFACE a. The amount of new impervious surfaces on the portion of a lot or parcel that is within 300-feet of the Ordinary High Water Mark is limited to the following: b. Impervious surface is allowed on up to 15% of the lot with a Land Use Permit. c. Greater than 15% but no more than 30% impervious surface is allowed with a Land Use Permit and a Mitigation Plan per §17.30 PA, and §17.30 M. If greater then 15% then A stormwater management plan is needed and can be further discussed. Here is a link to the St. Croix County Code of Ordinances 1)i6 , } 7I < ~ $ Ii, c~{VG t)2 i~~i& '%11 1 k~7f Other items we discussed which I would like you to confirm was: • The old home has already been deconstructed and the only task remains is removing the foundation. • The outline of the total construction area does not represent soil disturbance or grading. • The soil area in the drainfield of the powts system will not be disturbed and no spoil pile will be placed within. • Silt fencing is going to be placed downslope of the construction area and immediate seeding will take place once the construction of the home/garage is complete. Please send me the calculations of the impervious surface along with the confirmation of the above bullets.
