TORMS, it has been determined that a significant number of patients
in the St: Croix.County Hospital essentially require medical and social care
that can be provided in an Intermediate Care Facility and/or a Skilled Nursing
Facility; and
Fd[iMAS, Ft is not feasible to remove these patients clue to a lack of
alternate care resources in the community with the result that they most remain
in the present facility_ and
WREREAS, it therefore appears necessary and desirable thatprrrtions
of the St. Croix; County Hospital be converted to an Tntermediate Gare Facility
and/or Skilled Nursing Facility, and
WHEREAS, .it is anticipated that the £t. Croix County Community Mental
Health, 'iental Retardati.or., Alcoholism and Drug .Abugp Isoard, created pursuant
to Section 51,42 'U''gconsl.,n Statutes TALI enter into contractural or other relationship
ulith the trustees of tate St. Croix Health Care facilitic! to utilize the
faci.:lit es of thie St. f!rofx Peal'th Center to fulfill its program,

St. Croix lea°l.th Cert-c be designated ars a'ntertaed.ate Care rAcility .and/or Skille4
'lursing Facility and that the Superintendent of St. Croix Epalth Center be
instructed to apply for necessary licenses from the State of 'lisnonsin; and
IF IT PT.T'RTRER R.E50LVED that a portion of the new building facilities
of the hospital located at St. Croix Health Center, said aorti.oat consisting;
of 14 beds in the Intensive Care Section be designated as facilities for
the operation of a Community Mental Health In-Patient Services tnit, and
the Superintendent of St. 'Croix Health Center is directed to apply ,for necessary
State of Wisconsin approval as required, and
BF IT ?+UPTHER RESOLVED that the present Trustees of the "t. Croix County
ospital be and hereby are elected and authorised to be Vie Trustees of the
Intert.ediate Care Tactlfty and/or Skilled Nursing Facility crestted by this
resolution, pursuant to Section 46.18 of the Wisconsin StRtute$; and
Th.%t the effective date of this resolution be January 1, 1974.
