Slats. The contents of the septic. tank shall be disposed of in accordance with NR 113, Wis. Adm. Code. The operating condition of the septic tank and outlet filter shall be, assessed at least once every 3 years by inspection. The oullel liter shall tie cleaned as necessary to ensure proper operation. The filter cartridge should not be removed unless provisions arc made to retain solids in the tank that may slough off the filter when removed from its enclosure. If the filter is equipped with an alarm. the Filter shall be serviced it the alarm is activated continuously. Intemndtent filter alarms may mdicale, surge flows or an impending continuous alarm. The septic. lank shall have its contents removed when the volume of sludge and scum in the lank exceeds 1x3 the liquid volume of the tank. I visited all four lots for this CSM to verify data submitted on Real Prop March 161h amended soil reports. With your assistance in re-flagging some of the 7>s - 677 test pits and hand borings, I was able to confirm locations for all tested areas. I apologize if previous memos made it appear that work had not been completed, but Re IcLn„ the discrepancies between lot lines for concept and final CSM, the surveyor's soil -386-4675 boring markers on the map, plus the use of two test pits by Dave Fogerty all made it very difficult to identify the tested areas. Lot 1: There are two soil reports for this lot. The BM#2 for the 3/13/06 revised site plan was not visible anywhere near B2, but BM#1 is flagged in blue and available for use in mound installation. The tested area on soil report dated 10/23/03 has pink flagging and reported A+0" conditions, which would require a state on-site soil determination for mound design approval. See requirement for Lot 4. Lot 2: There are also two valid soil reports for this lot. Your BM#1 is a nail on west side of tree 21 feet south of B2 and flagged with blue tape, as are B1 and B2 (both excavated pits). BM#2 is the "top of PVC pipe" at 23" above grade and the 98.78' elevation should be double-checked. Dave Fogerty's soil report extends south of Mary Jo's tested area and allows flexibility in mound location and design. The site has not been altered and the contours indicated adequately represent the existing topography. Lot 3: My only criticism is that Comm 85.20(2)(b) specifically requires "at least one soil profile evaluation excavation per treatment or dispersal site shall be constructed as a soil pit" for estimated DWF of <1,000 gallons per day. This lot has three hand borings, with B3 being a hand-auger hole that was done while frost was still in the ground. The nearest excavated test pit is Dave Fogerty's B1, which is on Lot 2 >40 feet away. The new hand boring B3 is 77 feet southeast of a flagged as B1, which is 16 feet south of the north lot line. The "existing pit from Fogerty" (131) is 41 feet northeast of your B1. The PVC pipe shown as BM#2 has been flagged in pink tape and B2 is flagged next to it. Several hours of staff time were required to identify /iccycGq{ that there were discrepancies between the final lot line configurations and the 715-386-4675 reported tested areas, which was in part due to the lack of data provided on the original soil report plot plans. I reviewed the amended reports and comments are summarized as follows: Lot 1: The soil report dated 1;25:04 did not have a distance indicated from Boundary Road, rather it referenced a "proposed east property line". This was mistakenly assumed to mean the current Lot 1 configuration with Boundary Road right-of-way as the east lot line, but you submitted a concept map that showed locations for 2 lots that had been aroposed prior to final CSM. I will be happy to revisit this site to verify data on plot plan that now shows this site is 353 feet west of road. However. BM#2 is still shown at elevation 106.0' and needs a corrected elevation for use in construction of a POWTS. As mentioned in previous memo, Site #1 tested area (soil report dated 10.!23?03) had A+0" conditions and would require a state on-site soil determination for mound design approval. This area was flagged and still has benchmarks as indicated, although BM #2 elevation is incorrectly calculated. Lot 2: Again. I will re-visit the site to verify that your original borings and BM#1 are visible for POWTS installation purposes. I checked the area north of BM#2 twice on 3.'3!06 and found some blue flagging on a tree branch, but could not specifically locate BM#1 or B1 (59" deep boring) or B2 (50" deep boring). The southerly test pit and BM#2 shown on the plot plan match the locations of Dave Fogerty's B-2 and alternate benchmark, the latter "top of PVC pipe" is 23" above grade and should be higher than the 98.78' elevation reported. Corrected elevation is needed. Dave Fogerty's soil report extends south of Mary Jo's tested area and allows flexibility in mound location and design. Lot 3: The amended plot plan now includes a new hand boring B3 that is approximately 85 feet southeast of your B1. The' existing pit from Fogerty' 81 is shown and Fogerty's original BM#1 are still included for an assumed 100' elevation. The PVC pipe shown as BM#2 was previously observed and I will verify location of new B3 in relation to the staked north lot line. Upon completion of corrected elevations for alternate benchmarks and data verification, these soil reports should be acceptable for use in POWTS design. It is not uncommon for proposed lot configurations to change after soil testing has been completed. but review of submitted soil reports and on-site inspections help county staff identify discrepancies before plumbers use the data for POWTS designs. It is not always obvious (on paper) that the tested area(s) may not meet code requirements. Anytime more than one soil report is completed for a parcel, especially if there is overlap in the tested areas. errors andfor confusion of the data are possible. In this case, referencing another soil tester's data without properly identifying that two different borings were used caused additional confusion regarding site conditions. All borings and site reference data must be identified and elevations must be correlated using one primary benchmark. Even though it created extra work for both the soil tester and county, it's good that the request for site condition verification was made early enough to identify and correct problems prior to POVVTS design and installation. Stating or documenting observations made as a result of site condition verification is necessary so that any errors or discrepancies can be corrected. If data can't be readily confirmed at the site based on the soil report plot plan and soil descriptions, it's the government unit's (county) responsibility to notify all parties and obtain necessary clarification and~or corrections. Nobody wants to discover errors after a POINTS has been designed using incorrect soil report data. The focus here is to provide the landowner and prospective buyers with viable soil reports for each lot in the CSM. This memo was a waste of taxpayer's monies. A phone call to this soil tester since there were difficulties locating soil borings could have cleared this up immediately. A copy of the memo was also sent to the owners that hired me to complete the job. This memo appears to apply that the job was not complete. The only problem that there appears to be is that property lines were not established at the time the soil testes were conducted and they are not exactly were they were determined by the developer and had been changed between the times the tests were conducted. I am a licensed professional that has been licensed for almost 18 years and have been in business successfully for 10 years. I take my license and business very seriously along with the health of the public and groundwater of the State. Also referenced in the memo were soil pits that were excavated that was not found by the Inspector and it was insinuated that maybe they were not completed as reported. This is slander to my character and reputation. We are supposed to be working as a team, not as suspicious characters. These lots were discussed at the time the job was being conducted with Kevin Graubau, who was out on the site initially with Leroy Jansky because I did find the three pits that were left open. It was discussed if a County on-site was required and it was determined that if the soils were similar to the open pits that it was not required. These reports were also reviewed before lot approval. It seems a little redundant to have three personnel members reviewing the same reports. I have spent another 3 %z hours reviewing the site and paperwork. Here are the facts and findings and revisions to the reports: 1 Page 2 - Property Owned by Brent and Sonja Johnson, Town of Troy LOT 1 : Two tested sites are on this lot. They were not located correctly on the engineering/surveying map as my paperwork showed proposed lot lines that were changed. Site #1 completed in 2003 was located on your site visit. Apparently you had the paperwork for site #2 that was completed in 2004. 1"hat site was reported correctly based on proposed lot lines at that time. I have submitted the current lot lines. LOT 2: The lot line between Lot 2 and Lot 3 has been established. Based on my findings, the only 71;-386-467' currently viable soil report for use in the design of a POWTS was done by Dave Fogerty in September 2003, whose tested area is entirely on lot 3. Findings are xcc rli" summarized as follows: 15-386-467; Lot 1: located one tested area from soil report dated 10123103, which had A+0" conditions and would require a state on-site soil determination for mound design approval. This area was flagged and still has benchmarks as indicated, although BM #2 elevation is incorrectly calculated. A different tested area was described on a soil report dated 1!25'04 in the northeast corner of the lot. BM#1 is shown as 115' west of Boundary Rd. and 120' south of the north lot line, with three excavated backhoe test pits within 90 feet of it. There are no markers or visible backfilled pits to identify this area and the slope shown on the report is reversed from the topography I observed. Until the site on the more recent report can be identified and the data verified by county staff, no sanitary permits can be issued for this lot. Lot 2: Dave Fogerty's soil report for this lot was done prior to CSM; all three lest pits and both benchmarks are still visible and the elevations using BM#1 as assumed 100' elevation were found to be accurate. This site had an on-site completed by Leroy Jansky, Dept. of Commerce, who agreed that a mound or at- grade system was feasible based on soil conditions. The site has not been altered and the contours indicated adequately represent the existing topography. Your subsequent soil report (5.`504) utilized one of Dave's test pits and indicated that the south lot line had been staked by owner. The official surveyed lot line and corner stake are farther south than the flagged lathe that appears to be the owner's marker. The southerly test pit and BM#2 shown on the plot plan match the location and elevations of Dave Fogerty's B-2 and alternate benchmark of 2" PVC pipe, which are 145 ft. north of the south lot line. I could not locate your backhoe test pits B1 or B2, or BM#1 that are indicated as north of Dave's test pit. However, the north lot line is 50 feet farther south than your plan shows; there is a surveyor's stake and lathe marked "lot 2" at the SW lot corner for reference. I found the 1" PVC pipe shown as BM#2 on the plot plan and the elevationsfslope data appear correct, but no visible sign of hand borings or markers indicating B1 and B2 locations. This soil report is not useable since two of the three test pits are not on this lot. Although your soil reports for lots 2 & 3 make it look like only one of Dave's test pits was utilized. the data indicate that you mapped tested areas using both his B-1 and B-2. Your test pits excavated to 59" and 50" on the lot 2 report should have resulted in backfill material or other disturbance that would be at least as visible as the exposed test pits from Fogerty's soil report. I re-visited the site to try and locate these pits without success. Neither your Lot 2 or Lot 3 soil reports will be accepted for use in POWTS design or sanitary permit issuance. A copy of this memo will be attached to the soil reports and sent to the owners.
