Over the course of several ,dears. the property has been cleared of vegetation and existing grades
have been disturced Emergency storm, -rater management and repairs were installed in 2017 to
contain r,.noff and prevent sedimentation into Squaw La<e. This permit approvai is to restore the
Site.
• A floodplain analysis determined the Regional Flood Elevation (RFE) for Squaw Lake in in 2015. The
till :violation was below the RFC however, it was determined it did rot affect the RI L; attached is letter
dated June 4, 2015 fro r Wisconsin Department of Natural Resources

Community Development staff have reviewed the after-the-tact Land Use Permit application for the
deposition of till (2013 violation) in the Floodway Overlay Zoning District pursuant to Chapter 17.4
13A. and for land disturbance activities pursuant to Chapter 1117 in the Shoreland Overlay Zoning
District.
Over the course of several years, the property has been cleared of vegetation and existing grades
have been disturbed. I rnergency storm water management and repairs were installed in 70'17 to
contain runoff and prevent sedimentation into Squaw Lake. This permit approval is to restow the
site.
• A floodplain analysis determined the Regional Flood Llevation (WE) for Squaw I ake in in 2015. The
fill violation was below the RFE however, it was determined it did not affect the RFF; attached is letter
dated June 4, 7(115 irorn Wisconsin Department of Natural Resources.
• A wetland delineation was submitted on July 2(1, 2018.
• Best Management Practices and Storm water Management will be incorporated with the restoration
of this silo.
• The Private Onsite Wastewater Treatment System (POWTS) tailed, In 2011 a Sanitary Permit was
approved for an additional septic tank to be placed for ternpotmy use as a holding lank. A
replacement drainfield is sill necessary.
• This approval is for restoration of the site only. no new structures or land disturbance activities other
than to restore the site are permitted.
Based on these findings, approval of the Land Use Permit is subject to the following conditions:
This permit only approves the land disturbance activities necessary to restore the site.

control measures and to verify the accessory structure has been rernoved. Please call me. at (!1S)

6. It is the applicant's responsibility to secure any other required local, state or federal permit(s) and
approval(s) pnor to land disturbance activity.
This approval is subject to the corditions listed above; it does not allow for any additional
construction, structures, or buildings beyond the limits of this request. Your information will rernarn
on file at the St Croix County C ornmunity Development Department suite. It is your responsibility
to ensure compliance with any other local, stale, or federal permitting or regulations, Including
contacting the Town of Star Prairie and the Department of Natural Resources to inquire if additional
permissions are required.
Iftis permit is valid for one year, with the possibility of up to two (2) six month extensions if the
applicant suhmits the appropriate permit extension fee and doarmentation to the Zoninq
Administrator. A copy of the Land Use Permit placard should be submitted to the local Building
Inspector for the town upon application for the building permit, The orange placard must be posted
on thejob site and visible frorn public view.

'I lie Department of Natural Resources has reviewed the tloodplain analysis submitted by Joseph
Pingel. P.F.. of Cedar Corporation., on behalf of Mr. Edward richenor, located in Township 11 North,
Section 8, Range 18 West, *]*own of Star Prairie, St. Croix County. Wisconsin.
The hydrologic and hydraulic analysis and memorandum regarding the floodplain analysis for Mr.
F.dward'fichcnor was submitted to the Department on May 7, 2015. The Department will consider the
assumptions applied in the analysis sufficient, therefore approving the study as complying with State
Administrative Code NR 116 technical standards for application of St. Croix County's Floodplain
Zoning Ordinance.
[lie analysis determined the Regional Flood Elevation (RI'E) for Squaw Lake, a seepage lake, to be
944.5 ft. NAVD88. The analysis and LIDAR information indicated the fill associated with Mr. Edward
Tichenor's property was assumed to be below the RFF,.Therefore impacts on the RPE were investigated
in IIEC-11D4S by Joseph Pingel, P.L. and determined not to have any impacts.

`L• GENERAL APPLICATION REQUIREMENTS
APPLICATION:
Applications will not be accepted until the applicant has:
• Met wan the Zoning Administrator to review the application;
• Submitted Orsinal Plus 2 copies of the entire Packet including all supporting information, maps & diagrams:
• Resolved any land use violations and paid any outstanding fees owed to the Community Development Department;
• Signed the application form (the signatures of the property owners and agents acting on their behalf ore required); and
• Submitted the appropriate application fee (nonrefundable) payable to: St. Croix Count
v.
REVIEW:
The Zoning Administrator will review the application for completeness and assign a file number to the application. The Zoning
Administrator may require additional information and will notify the applicant of this within 10 days. Upon receiving a complete
application and supporting documents, the Zoning Administrator will:
• send copies of the applications to the appropriate reviewing agencies for comment. Applicants are encouraged to contact
their town to discuss their application and inquire about necessary building permits and approvals at the town level;
• schedule a site visit to the applicant's property, at which time the applicant shall flag all applicable property/project corners
and label the flags accordingly; and
• review the file and prepare findings for approval or denial of the permit within 60 days. Upon approval, the permit will be
ri to the applicant or to the applicant's agent.
If approved, the land use permit will be valid for one year from the date of the permit issuance. The applicant may also be required
to apply through the Town for a local building permit. All site plans, pictures, and other materials submitted with the application
become the property of the Cornmunit; Development Department and will remain in the file

• location of all existing and proposed structures with their square footage and distance from setbacks;
• location of existing and proposed POWTS, wells, driveways, parking areas, access, signs, and other features;
• location of slopes 12% and greater (minimum contours to be determined by the Zoning Administrator);
• all blufflines and slope preservation zones (Riverway) and setbacks from blufflines;
• location of the OHWM of any abutting navigable waterways and ali setbacks from the OHWM;
• location and landward limit of all wetlands, specifications and dimensions for areas of proposed wetland alteration;
• existing and proposed topographic and drainage features and vegetative cover;
• location of floodplain and flood-way limits on the property; and
• any other unique limiting conditions of the property or information deemed necessary by the Zoning Administrator.
For projects that involve land disturbance requests, the following additional information may be required:

❑ Grading plan showing grading limits and pre and post contours.
❑ Project schedule and contractor list.
❑ Erosion control p';an (Best Monagement Practices).
❑ Storm water management plan stamped by an engineer and including ail runoff calculations.

Narrative
The Tichenor property is located on the east side of Squaw Lake in Star Prairie at 2236 100"
Street and 2234 1001" Street. This project includes restoring the site, including the delineation
and restoration of wetlands, grading, storm water management, erosion control, and a new
septic system.
The owner of the property cleared the land of all vegetation and modified the existing grades.
Citations have been issued by the WisONR and St. Croix County for impacts to wetlands, lack of
storm water management and erosion control, and impacts to the septic system. Emergency
repairs were performed about a year ago to prevent sediment and untreated water from
entering Squaw Lake as shown in Figure 1.
