RESOLUTION TO RELEASE COMMUNITY AIDS AND LOCAL TAX LEVY
FUNDING TO HELP FUND LONG -TERM CARE REFORM

St. Croix County, Wisconsin
WHEREAS, Wisconsin's Governor and Secretary of the Department of Health and
Family Services have proposed establishing a statewide managed long -term care system
for older people and adults with disabilities by 2011; and
WHEREAS, the redesigned managed long -term care system would serve all eligible
consumers under a capitated (per person per month) payment system without the current
waiting lists for services within two years; and
WHEREAS, a model redesigned system of managed long -term care reflects the following
values:
• Choice — Provide individuals with better choices of services, supports, and
providers,
• Access — Improve individuals access to quality services regardless of where they
live,
• Quality — Improve quality by focusing on individually identified and supported
health and social outcomes, and
• Cost - effective — Create cost effective solutions by managing to achieve quality
individualized care; and
WHEREAS, the Wisconsin Department of Health and Family Services made planning
and implementation grants available to support achievement of this managed long -term
care system; and
WHEREAS, St. Croix County is one of nine counties and three private partners
comprising the West - Central Wisconsin Care Management Collaborative (WCW -CMC)
which was awarded a $250,000 grant in 2005 to plan and implement managed long -term
care in the western region of the State of Wisconsin; and
WHEREAS, St. Croix County Board of Supervisors adopted resolution 8 (2007) which
resolved that St. Croix County elects to not create a new licensed Care Management
Organization that would bear the financial risk for all Long Term Care covered services
and that St. Croix County is ready with other area counties to request that the Wisconsin
Department of Health and Family Services issue a Request for Proposal for a Care
Management Organization to provide redesigned Long Term Care services in our area.
WHEREAS, it was further resolved that the St. Croix County Department of Health and
Human Services and the St. Croix County Department on Aging will continue to work
cooperatively on establishment of an Aging and Disability Resource Center.

WHEREAS, the Legislature has determined that to financially support regional managed
care and a network of local Aging and Disability Resource Centers each county involved
in the redesigned system of long -term care will contribute the amount of state
Community Aids and county levy that it spent on adult MA Waiver cases in the year
2006 and that the contribution over a period of five (5) years will be reduced to 22% of
that county's Community Aids; and
WHEREAS, St. Croix County's contribution of Community Aid and County Levy in the
first program year will be approximately $2,669,902 and by the fifth and for future years
will be approximately $376,008, or 22 %, of our St. Croix County's Community Aids
Allocation; and
NOW THEREFORE BE IT RESOLVED that the St. Croix County Board of Supervisors
hereby determines that establishing a regional managed long -term care system, including
local Aging and Disability Resource Centers is in the best interests of St. Croix County
residents and, therefore, approves the release of Community Aids and local tax levy
funding, as eligible cases are transferred from St. Croix County to the regional care
management organization.
