HEALTH AND HUMAN SERVICES BOARD
FUNCTIONS TO ADMINISTER THE
AGING AND DISABILITY RESOURCE CENTER

WHEREAS, the St. Croix County Board of Supervisors adopted Resolution No. 8 (2007)
regarding long -term care redesign in St. Croix County on April 17, 2007; and
WHEREAS, Resolution No. 8 (2007) states that St. Croix County will establish an aging
and disability resource center (ADRC) that: (1) is a trusted source of information about
prevention strategies, community resources, and long -term options for the elderly or
persons who have a disability; (2) serves persons whether or not they have resources to
purchase their own services or need help doing so; (3) serves as a single entry point to
publicly- funded long term care (LTC) programs; and (4) would have a role in ensuring
that consumers' needs are met by managed care organizations. The mission of an ADRC
is to empower and support seniors, persons who have a disability and their families by
providing useful information and finding help that persons seek through in person visits
to the resource center, telephone inquiries or visits to the consumer's home; and
WHEREAS, the St. Croix County Board of Supervisors adopted Resolution No. 24
(2007) which authorized the Department on Aging to submit an application for an
ADRC; and
WHEREAS, because of long term care reform, an Aging and Disability Resource Center
(ADRC) is being created by St. Croix County; and
WHEREAS, the creation of the ADRC has made it necessary to restructure the
Department on Aging and the Department of Health and Human Services; and
WHEREAS, the Council on Aging and the Health and Human Services Board have
approved an organizational structure, governance and key commitment document for
implementation of the ADRC as signed by committee representatives and agency staff
(see attached); and
WHEREAS, Article IV, Section A.3.f. of the St. Croix County Board of Supervisors
Rules and Bylaws will be renamed the Council on Aging and Disabilities and the makeup
of the appointed committee will be changed to consist of 4 older adults over 60, 2 of
whom are supervisors, 2 members of the Health and Human Services Board who are over
60, one of which is a supervisor, 1 caregiver for older adults, 1 representative with
knowledge in the area of developmental disabilities, 1 representative with knowledge in
the area of physical disabilities, 1 representative with knowledge in the area of alcohol
and drug abuse /mental health, and 1 representative with knowledge in the area of youth
transition; and
WHEREAS, in the St. Croix County Board of Supervisors Rules and Bylaws Addendum,
Committees and Descriptions, the Council on Aging will be renamed the Council on
Aging and Disabilities and the descriptive paragraph will read as follows:
The responsibilities of this committee are to receive, disperse and administer
funds for services and programs for older persons, and assist with organizing,
developing, and expanding available services and programs for older people and
persons with disabilities. Determine the structure, policy and procedure of the
ADRC within State guidelines and local governance structure.
THEREFORE BE IT RESOLVED that the St. Croix County Board of Supervisors
approves the committee's reorganization and directs the changes in the rules and bylaws
be made as set out above.

Governance:
Consistent with the requireinents outlined in the state application for ADRC funding the
governing board of the ADRC +M!1 be madc up of a maximum of 11 members. 50% of which are
consumer representatives. As the oversight body for Older Americans Act funding, a minimum
number or 50% of this board will be 60 years or older. Board members will apply and he
appointed by the County Board Chair to achieve. the following profile;
4 — older adults. 2 of whom are currently serving on the Council of Aging
2 — members of the I Teak!) & Human Services Board (who arc agc 60 or older)
1 — caregiver for older adults
— representative with special knowledgeiexpertise in the area of developmental disabilities
1 — representative with special knowledge/expertise in the area of physical disabilities
1 — representative with special knowledgelexpertisc in the area of AODA/Mental Health
1 — representative %vith special knowledge/expertise in the arca of youth transition
Key Com m it men ts:
• Health & Iluman Service Board hy-laws will be amended to require 2 members of the HHS board
to be a person 604
• Strive to have irpresentation from throughout the county on governing board
• COAD will continue to have oversight responsibility for Older America AC-1 ati well as
monitoring programs and services related to Aging and Disability Resources
• ADRC revenues (100% of the Fed Matching funds, OAA funding, 85.21, donations, grants, tie)
gathered by A DRC stair will remain in the ADRC to support operations/expanded services
• ADRC Director and Council on Aging & Di iIi1ie (COAD) will have an opportunity annually to
present budget recommendations to Finance Committee after presentation to HMS Director
• COAD will have input in the hiring/tiring and evaluation of the ADRC Director
• ADRC Director will have opportunity to make annual report presentation to full county board as
well as the COAD
• The (:CAI) will have opponunity to create its own advisory coromittees as a means of involving
more elderly and people with disabilities in the governance process
• New ADRC Director will serve on management team at HHS. allowing greater opportunity to
promote visibility to aging and disability issues
• .1111S will provide. technical assistance to gct 100% turn O reporting function up and running as soon
as possible and accounts receivable office will be responsible for billing
