Community Development staff have reviewed the Land Use Permit application for the construction
of it single-family dwelling, attached garage, and driveway in the Shomland Overlay Zoning District
pursuant to chapter 'I i.30 I2.a. The request has been conditionally approved based or, the
apolication submission and the following findings. Please see the e-closed site clan to relevance
the general erosion control and grading notes.
• I he 15 foot setback to the ordinary high water mark has bear, rnel
• I he setbacks to the properly lines have been met.
• Iwal land disturbance is approximately 10,000 square feet.
• Erosion and Sediment Control plans have been submitted that meet Wisconsin Department o`
Natural Resources Technical Standards. Lied Management Practices and stonnwater management
will be incorporated.
• The Impervious Surface (IS) calculation was pertol and resulted in 11.17%, mitigation will not be
required.
• The County sanitary permit for a new Private Onsite. Wastewater Treatment Syslern Is currently under
revlew.
• No land disturbance to slopes greater than 19.99% is to occur.

1. Prior to Construction, the 15 foot shoreland setback must be flagged and erosion control measures,
such as silt fencing must be installed downslope the site prior to land disturbance.
2 A pre-construction inspection is required to validate setbacks and verify erosion and sediment
control has been installed downslope of construction activities. Please call me at (715) 380-4742 to
schedule this inspection.
3. St. Croix County reserves the right to require additional sediment and erosion control measures to
be installed if found necessary due to site specific cone erru and will he documented in an as-built
site plan.

4. All temporary erosion control measures shall be left in place and maintained until the site has
reached a point of at least 70% permanent vegetation. Permanent vegetation shall be established
once final grade is reached or as soon as applicable per plan.
5. A post-construction inspection is required prior to removing the lernporary sediment and erosion
control measures, please call me at (7'5) 386-4742 to schedule this inspection.
6. It is the applicant's responsibility to secure any other required local, state or federal permit(s) and
approval(s) prior to land disturbance activity.

This approval is subject to the conditions listed above. it does not allow for any additional
construction, structures, or buildings beyond the limits of this request. Your information will remain
on file at the St. Croix County Community Development Deparlrnent suite. It is your responsibility to
ensure compliance with any other local, state, or federal permitting or regulations, including
contacting the town of Star Prairie and the Department of Natural Resources to inquire if additional
permissions are required. 1 his permit is valid for one year, with the possibility of up to two (2) six
month extensions if the applicant submits the appropriate permit extension fee and documentation
'o the Zoning Administrator. A copy of the Land Use Permit placard should be submitted to the
town's local Building Inspector upon applying for town building permit(s). The orange placard must
be posted on thejob site and visible frorn public view.
