WI 54013 RF Conditional Approval: I ilefl LUP-2018-073 Project Location: 34.30.15514, Town of Glenwood Project Address: 3020 County Rd DD David & Jennifer, Community Developrnent staff have reviewed the Land Use Permit application for the construction of a single farnily dwelling, attached garage, and driveway in the Shoreland Overlay Zoning District pursuant to chapter 17.30 12,a. The request has been conditionally approved based on the application submission and the following findings. Please see the enclosed site plan to reference the general erosion control and grading notes. 1 he /5-foot setback to the ordinary high water mark (01 IWM) has been met. 1 he 50-toot setback to the DNR mapped wetland has been rnet. The site is riot within the Zone A FEMA DFIRM Floodplain. • The setbacks to the property lines have been net. Total land disturbance within 1,000 feet of the 011WM is greater than 10,000 square feet. • No land disturbance to slopes greater than 19.99% is to occur. 1 rosion and Sediment Control plans have been submitted that meet Wisconsin Department of Natural Resources Technical Standards. Best Management Practices and stounwater management will be incorporated. • The existing single family dwelling is to be razed and no longer Utilized a5 a single-family residence. • The County sanitary permit for if new Private Onsite Wastewater Treatment System is approved, permit number 606989. Based on these findings, approval of the Land Use Permit is subject to the following conditions: 1. A pre-construction inspection is required to validate setbacks and verify erosion and sediment control has been installed downslope of construction activities. St. Croix County reserves the right to require additional sediment and erosion control measures to be installed if found necessary due to site-specific concerns and will be documented in an as-built site plan. All temporary erosion control measures shall be left in place and maintained until the site has reached a point of at least 70% permanent vegetation. Permanent vegetation shall be established once final grade is reached or as soon as applicable per plan. 4. A post-construction inspection is required prior to removing the temporary sediment and erosion control measures, please call me at (11S) 386 4742 to schedule this inspection. 5. The existing single-lamily dwelling can no longer be utilized as a single-family residence as only one single family-dwelling is permitted in the Rural Residential District. 6. It is the applicant's responsibility to secure any other required local, state or federal permit(s) and approval(s) prior to land disturbance activity. Failure to comply with the terms or conditions above may result in the revocation of this permit by the 7oning Adrrunistrator pursuant to Chapter 17.71 and chapter 17.30(13). this approval is subject to the conditions listed above; it does riot allow for any additional construction, structures, or buildings beyond the limits of this request. Your information will rernain on file at the SL Croix County Community Development Department suite. It is your responsibility to ensure compfance with any other local, slate, or federal permitting or regulations, including contacting the lown of Glenwood and the Department of Natural Resources to inquire if additional permissions are required. I his permit is valid for one year, with the possibility of up to two (2) six- month extensions if the applicant subrruts the appropriate permit extension fee and documentation to the Zoning Administrator. A copy of the Land Use Permit placard should be submitted to the town's local Building Inspe(tor upon applying for town building perrnil(s). The orange placard must be posted on the job site and visible from public view.
