APPLICATION:
Applications will not be accepted until the applicant has:
• Met with the Zoning, Administrator to review the application;
• Submitted Original plus 2 copies of the entire packet including all supporting information, maps & diagrams;
• Resolved any land use violations and paid any outstanding fees owed to the Community Development Department;
• Signed the application form (the signatures of the property owners and agents acting on their behalf are required), and
• Submitted the appropriate application fee (nonrefundable) payable to: St. Croix County.
REVIEW:
The Zoning Administrator will review the application for completeness and assign a file number to the application. The Zoning
Administrator may require additional information and will notify the applicant of this within 10 days. Upon receiving a complete
application and supporting documents, the Zoning Administrator will:
• send copies of the applications to the appropriate reviewing agencies for comment. Applicants are encouraged to contact
their town to discuss their application and inquire about necessary building permits and approvals at the town level;
• schedule a site visit to the applicant's property, at which time the applicant shall flag all applicable property/project corners
and label the flags accordingly; and
• review the file and prepare findings for approval or denial of the permit within 60 days. Upon approval, the permit will be
mailed to the applicant or to the applicant's agent.
If approved, the land use permit will be valid for one year from the date of the permit issuance. The applicant may also be required
to apply through the Town for a local budding permit. All site plans, pictures, and other materials submitted with the application
become the property of the Community Development Department and will remain in the file.

• location of existing access roads. eight of way, road setbacks, and recorded easements;
• location of all existing and proposed structures with their square footage and distance from setbacks,
• location of existing and proposed POWTS, wells, driveways, parking areas, access, signs, and other features;
• location of slopes 12% and greater (minimum contours to be determined by the Toning Administrator);
• all bluf flines and slope preservation zones (Riverway) and setbacks from bluff ine%;
• location of the OflWM of any abutting navigable waterways and all setbacks from the OHWM;
• location and landward limit of all wetlands, specifications and dimensions for areas of proposed wetland alteration;
• existing and proposed topographic and drainage features and vegetative cover;
• location of floodplain and floodway limits on the property; and
• any other unique limiting conditions of the property or information deemed necessary by the Zoning Administrator.

STATEMENT OF SEPTIC OPERATIONS

located on the property described as follows

This system was pumped and inspected on January 19, 2018. The holding tank is steel and
appears to be water tight. The cover needs to be locked. The alarm doesn't work.
Owner is responsible for disclosing all known septic issues including surface discharge.
This is not a guarantee to the longevity of the system. This statement is an opinion, not a
warranty or guarantee, and I disclaim all liability for any loss caused by the reliance on this
statement.

Community Development staff have reviewed the Land Use Permit application for reconstruction of an
existing nonconforming principal structure. The request has been conditionally approved based on the
application submission and the following finding,.
• The reconstruction of existing principal structure will be within the existing footprint. The foundation
will not be replaced; portions of exterior walls and the roof will be replaced. The setbacks for the
principal structure will remain unchanged.
• The height of the reconstructed principal structure does not exceed the existing structure height. The
current height of the structure, measured frorr the roof overhang riverward to the existing foundation
of the eastward wall is 25-teet and 1-inch.
• No retaining walls or patios are proposed to be replaced.
• Storm water management and erosion and sediment control plans are submitted per § 17.36 H.7. No
land disturbance such as filling/grading to occur with the exception of installing permeable pavers
within the driveway as indicated on the site plan.
• All structures will remain screened and visually inconspicuous from the Riverway. No vegetation is
being removed between the principal structure and Riverway. (4) Four native trees of 2-inch diameter
breast height will be planted as indicated on the site plan to ensure the existing structure will be
visually inconspicuous from the Riverway.
• The existing stairs and decking will not be replaced. The agent was informed the property owners
would need to obtain a variance for any work to the existing stairs and decking.
• The existing private onsite wastewater treatment system (POWTS) was inspected January 22d, 2018
by Marko Septic, Ll C. The cover needs to have a lock installed and the alarm to the holding tank needs
to be replaced.

Rased on these findings, approval of the Land Use Permit is subject to the following conditions:
1.A pre-construction inspection is required to verify the reconstruction of the principal structure meets
the site plan and the above findings are upheld. Additional inspections may be. scheduled as the project
commences.
2.St. Croix County reserves the right to require additional sediment and erosion control measures to be
installed if found necessary due to site specific concerns and will be documented in an as-built site plan.
3.A post-construction inspection will include inspection of; the permeable paver installation, the
reconstruction has met the site plan, native plantings of 4 native trees of 2-inch diameter at breast
height. In addition, a copy of the order form indicating the type and quantity of permeable pavers is to
be provided by the contractor. The installation Outline of the permeable pavers is to also he collected
at this time from the contractor. Failure to obtain this documentation will result in violation of § 17.36
11 7 and this permit will be revoked.
4. An inspection of the POWTS by our del,arlrnenl and a written verification by a certified plumber is
required to ensure the rover lock and alai rn is replaeeu
S.It is the applicant's responsibility to secure any other required local, state or federal permit(s) and
approval(s) prior to land disturbance activity.
6.The property owner shall execute and record an affidavit with the St, Croix County Register of Deeds
desrnbmg the approved storm water management plans and maintenance requirements within 30 days
after construction is completed.
7.Failure to comply with the terms or conditions above may result in the revocation of this permit by the
Zoning Administrator pursuant to Chapter 17.71.

i his approval is subject to the conditions listed above; it does not allow for any additional construction,
structures, o, buildings beyond the limits of this request. Your information will remain on file at the St.
Croix County Community Development Department suite. It is your responsibility to ensure compliance
with any other local, state, or federal permitting and regulations, including contacting the Town of St.
Joseph and the Department of Natural Resources to inquire if additional permissions are required.

This permit is valid for one year, with the possibility of up to two (2) six-month extensions if the applicant
submits the appropriate permit extension fee and documentation to the Zoning Administrator. A copy of
the Land Use Permit placard should be submitted to the local Building Inspector for the town upon
application for the building permit. The permit placard must be posted on the jobsite arid visible from
public view.
