HomeMy WebLinkAboutResolution 2007 (08) • RESOLUTION REGARDING LONG TERM CARE REDESIGN IN ST. CROIX COUNTY Resolution No. (Z 1 St. Croix County, Wisconsin WHEREAS, Wisconsin's Governor and Secretary of the Department of Health and Family Services have proposed to redesign the delivery of Medicaid funded Long Term Care services to the frail elderly and adults with physical disabilities or developmental disabilities into a person- centered care management system designed to maximize the ability of individuals and their families to maintain or improve their health and independence; and WHEREAS, the redesigned care management system aims to serve all eligible consumers under a capitated (per person per month) payment system without Waiting Lists for services; and WHEREAS, a model redesigned system of care should reflect the following values of: • Choice — Provide individuals with better choices of services, supports and providers, • Access — Improve individuals' access to quality services regardless of where they live, • Quality — Improve quality by focusing on individually identified and supported health and social outcomes, and • Cost - effective — Create cost effective solutions by managing to achieve quality individualized care needs not simply for cost; and WHEREAS, the redesign initiative being undertaken represents more than a decade of effort in this State to improve and enhance the current long -term care system, which includes five Family Care pilot counties and four private not - for -profit Partnership demonstration projects; and WHEREAS, a consortium of public and private partners in the area, including nine counties and three private not - for - profit entities, was awarded a state grant to explore a redesigned integrated health and long -term care system; and WHEREAS, because of our commitment to individualized quality services to all eligible consumers in a system without Waiting Lists, St. Croix County representatives have been actively involved in the consortium considering the available options for redesigning Long Term Care; and WHEREAS, the St. Croix County Long Term Support Committee is aware of local planning and is in support of reform efforts to remove Wait List; and WHEREAS, St. Croix County will establish an Aging and Disability Resource Center that 1) is a trusted source of information about prevention strategies, community resources, long -term options for people who are elderly or have a disability, that 2) serves all people, whether or not they have resources to purchase their own services or need help doing so, that 3) serves as the single entry point for publicly - funded LTC programs, and 4) would have a role in quality assurance to ensure consumers' needs are met by the care management entity. The mission of ADRC's is to empower and support seniors, people with disabilities and their families by providing useful information and finding the help people seek. Personalized assistance is available at the resource center(s), over the telephone, or in visits to an individual's home; and { l • RESOLUTION REGARDING LONG TERM CARE REDESIGN IN ST. CROIX COUNTY (Continued) Resolution No. WHEREAS, a major policy decision for the County to consider is whether it does or does not desire to create a new licensed Care Management Organization and to assume the financial risk for all covered services; and WHEREAS, the recommendations from Department of Health and Human Services and County administrative staff are that St. Croix County elects to not create a new licensed Care Management Organization and to not bear the financial risk for all covered services; and WHEREAS, there are several experienced private not - for -profit entities in the area who could be the CMO's; and WHEREAS, St. Croix County encourages competition in the open procurement process for the request for proposal and has sent a letter to that effect to Secretary Hayden dated 3/21/07; and WHEREAS, St. Croix County will continue to negotiate with the State to minimize county contribution to financing the reformed system and we will ask that serious consideration be given to bring St. Croix County into this reformed system with a contribution that is similar to what other counties experienced in Family Care expansion. When the County contribution is determined, an additional resolution will be developed for County approval that specifies the St. Croix County allocation for long term care reform for the benefit package approved by federal and state authorities; and WHEREAS, the anticipated fiscal impact of this action limits the County's fiscal risk for all covered services to all eligible Long Term Care consumers in the future; and WHEREAS, the state Department of Health and Family Services has established a process whereby they will release a competitive Request for Proposal for the Care Management Organization when notified by a group of Counties that they are ready for the RFP in their area; and NOW, THEREFORE, BE IT RESOLVED by the St. Croix County Board of Supervisors that St. Croix County elects to not create a new licensed Care Management Organization that would bear the financial risk for all Long Term Care covered services and that St. Croix County p sis- e ready with other area Counties to request that the state Department of Health and Family Services issue a Request for Proposal for a Care Management Organization to provide redesigned Long Term Care services in our area. BE IT FURTHER RESOLVED that the Department of Health and Human Services and Department on Aging will continue to work cooperatively on establishment of an Aging and Disability Resource Center. 2 RESOLUTION REGARDING LONG TERM CARE REDESIGN IN ST.
