Community Development staff have reviewed the Land Use Permit application for land disturbance
activities affecting slopes 12 - 19.9% pursuant to the General Mining Chapter 11.12 and the
Shoreland Overlay District Chapter 1/30 I.2a., for the construction of a Single Family Dwelling,
driveway and septic system. 1 he request has beer. conditionally approved based on the application
submission and the foliowing findings. Please see -he enclosed site plan to reference the general
erosion control and gracling notes.
• The 75 toot Ordinary I ligh Water Mark has been met for all structures including the driveway.
• The property line setbacks have been rnet.
• Erosion and Sediment Control plans have been submitted that meet Wisconsin Department of
Natural Resources Technical Standards. Best Management Practices and will be incorporated.
• The Private Onsite Wastewater Treatment System has been approved under County Sanitary Permit
#SAN-2018-241.
• The Impervious Surface (IS) calculation was performed and resulted in 45%.
• the Strornwater Management plans have been submitted that rneet the performance standards
described in Wisconsin Administrative Code NR 151 Subchapter III, Non-Ag Performance Standards.
1. A pre-construction inspection is required to discuss the construction tirnefine and verify erosion and
sediment control has been installed downslope of construction activities and the vehicular tracking
pad installed per Wis. DNR Standard 1057 prior to any land disturbance activity. If existing
bituminous driveway is to remain during construction activities, this can be used in lieu of the tracking
pad, provided sediment does not track of(site. Please call me at (71S) 386 4742 to schedule this
inspection.
2. Permanent vegetation shall be established once final grade is reached or as soon as applicable per
plan.
3. All temporary erosion control measures shall be left in place and maintained until the site has reached
a point of at least /0°,% perrnanent vegetation.

4. A post-construction inspion is required prior to removing the ttPorary sediment and erosion
control measures and to verify the accessory structure has been removed. Please call me at (715)
386-4742 to schedule this inspection.
5. It is the applicant's responsibility to secure any other required lo(-al, state or federal permit(s) and
approval(s) prior to land disturbance activity.
6. 1 ailure to comply with the terms or conditions above may result in the revocation of this permit by
the Toning Administrator according to Chapter 11.30 (13).
Ihis approval is subject to the conditions listed above; it does not allow for any additional
construction, structures, or buildings beyond the limits of this request. Your information will remain
on file at the St. Croix County Community Development Department suite. It is your responsibility
to ensure compliance with any other local, state, or federal permitting or regulations, including
contacting the Town of Star Prairie and the Department of Natural Resources to inquire if additional
permissions are required. This permit is valid for one year, with the possibility of up to two (2) six
month extensions if the applicant submits the appropriate permit extension fee and documentation
to the Loninq Administrator. A copy of the Land Use Permit placard should be submitted to the
local Building Inspector for the town upon application for the building permit. The orange placard
must be posted on the job-site and visible from public view.
