Community Development Department March, 2TO 2018 FileA LUP-2018-009 Haas Sons, Inc. 203 Last Birch St. Thorp, WI 54771 RE: Land Use Permit-Conditional Approval Shoreland Zoning District Project Location: 487 Highway 128; Town of Cady Jason, Community Development Staff have reviewed your Land Use Permit application for a temporary concrete half h plan( within the Shoreland Zoning District. The request has been conditionally approved based upon the following findings. Please read the conditions carefully, as they must be followed for the permit to be valid. 1. 1 he property is within the Shoreland Zoning District 2. There are two phases to be conducted for this concrete plant, this Land usP Permit is valid for phase 1, 3. As per site plan the location of the temporary batch plant is designated, in addition sill fencing and berm construction is being conducted in phase 1 and shown on the site plan. Rased on these findings, approval of the Land Use Permit is subject to the following conditions. 1. An Installation Inspection is required to verify the silt fence has been installed per plan prior to any land disturbance activity. Please call the inspector at (715) 386-4742 to schedule this inspection. 2. The OHWM setback is 75' However; the vegetative buffer is 35' landward from the OIIWM. This 35' from the OHWM is to not be disturbed and is the closest the silt fencing shall be installed. Any expansion of the existing access is prohibited towards Cady Creek. 3 St. Croix County reserves the right to require additional sediment and erosion control measures to be installed if found necessary due to site-specific concerns. 4 Permanent vegetation shall be. established once final grade is reached or as soon as applicable. A temporary cover crop such as oats, winter wheat or rye, OR, mulching may be necessary if the site is not stabilized by September 15, 2018 S, It is the applicant's responsibility to secure any other required local, state or federal permit(s) and approval(s) prior to land disturbance activity. 6. Failure to comply with the terms or conditions above may result in the revocation of this permit by the Zoning Administrator according to Section 17.36 J. 8.b. Your information will remain on file at the SI_ Croix County Community Development Department suite. It is your responsibility to ensure compliance with any other local, state, or federal permitting or regulations, including contacting the Town of Cady and the Department of Natural Resources to inquire if additional permissions are required. This permit is valid for one year, with the possibility of up to two (2) six-month extensions if the applicant submits the appropriate permit extension fee and documentation to the Zoning Administrator. The placard must be posted on the job site and visible from public view.
