COUNCIL ON AGING AND DISABILTIES MEETING
September 17, 2009
County Board Room, Hudson
CALL TO ORDER: Vice Chair Wentz called the meeting to order at 9:00 a.m. Pledge of
Allegiance was said.
ROLL CALL: Supervisors: Al Schrank, Gerald Peterson, Esther Wentz, and Sharon Norton -
Bauman; Councilmembers: Ramsey Lee, Michelle Chevalier, Kristen Johnson, and Lois Burri.
Also present were: Administrative Assistant Renea Aeschliman; Community Services Coordinator
Tracy Davis; Preventative/Nutrition Program Coordinator Randi Hanson; Caregiver Support
Coordinator Nancy Abrahamson; Health and Human Services Director Fred Johnson; Brad
Beckman ADRC Manager; County Board Chairman Roger Rebholz; County Administrator Chuck
Whiting; Nursing Home Administrator Frank Robinson, Registered Dietician Teresa Kvam; Duana
Bremer. Absent were Sue Nelson, Vera Grant and Judy Harrison.
ACTION OF PREVIOUS MINUTES: A motion to approve the minutes as corrected was made
by Supervisor Scrank and seconded by Councilmember Lee; motion carried.
DATE OF NEXT MEETING: Thursday, October 15 in the County Board Room of the St. Croix
County Government Center. Supervisor Norton- Bauman will review vouchers.
1. Monthly Financial Report: Beckman handed out financial information for the ADRC and the
Aging programs as of August 31, 2009.
2. Consideration of Claims and vouchers: Councilmember Johnson reviewed the vouchers. A
motion to accept the vouchers as presented was made by Councilmember Johnson and seconded by
Councilmember Lee; motion carried. Please see page 4 for a list of vouchers reviewed. Supervisor
Norton- Bauman will review next month's vouchers.
3. ADRC budtet for 2010: Johnson informed the Council there were no adjustments made on the
budget at this time. The 2010 state allocations were received for the Aging programs and the
difference between the budgeted amount and the allocated was minor.
4. Frank Robinson, St. Croix County Health Center Administrator: Robinson handed out the
2015 Strategic Plan/2010 Operational Plan for the Nursing Home. He highlighted key points within
the plan and the goals for the Nursing Home.
5. Teresa Kvam, Registered Dietician: Kvam introduced herself to the Council and explained her
role with the ADRC. She is a Registered Dietician through Public Health and contracts with the
ADRC to review menus, consult with ADRC staff on consumers with dietary needs or special diets.

She also attends meetings with the Nutrition Program Managers, Cooks, and the Nutrition Advisory
Committee. Kvam will also be utilized for staff trainings and policy /procedure work.
6. Unmet Needs Presentation by Councilmember Chevalier: Councilmember Chevalier spoke
to the Council regarding her views on unmet needs in the County. She had found information
gathered by a survey conducted by churches in New Richmond area. Through her research she
found Grace Place in Somerset, which is emergency shelter for individuals that are homeless.
Duana Bremer, Salvation Army /Grace Place, spoke to Council regarding the programming and
services provided through Grace Place. Bremer informed the Council that Grace Place is always
full and individuals may stay for 90 days. During their stay individuals are linked with services in
the area, whether it be transportation, mental health services, or other housing options. Through
Bremer's experience she has found there are good services available within St. Croix County, but
not enough. She identified transportation, mental health services, counseling for mental health and
AODA, and halfway houses as those services that need to be increased.
7. Review of Council on Aging and Disabilities By -Laws: Beckman handed out the Council on
Aging and Disabilities By -Laws for review. The By -Laws were revised after Beckman met with the
State for the one year ADRC review. The Council will need to vote on the by -laws at the next two
meetings.
8. Aging Updates:
• CountyAging Unit Plan 2010 -2012 — proposal Beckman informed the Council he has not
heard from the state regarding any changes that need to be made on the County Aging Unit
Plan.
• Caregiver Program
• Interfaith Volunteer Needs handout A list of the current Interfaith Volunteer requests was
given to the Council by Caregiver Support Coordinator Abrahamson.
• Caregiver Conference — Save the date Abrahamson reminded the Council the
Conference will be held on Friday, October 23` in New Richmond at WITC. An example
of the brochures was given to the Council and brochures will be mailed out when printing
is finished.
• DUAway Adult Respite program update Abrahamson reported that the Hudson location
continues to be full and more volunteers are needed to assist with daily activities.
• Transportation Program
° Transportation requests to use the ADRC vans for out of county trips were presented by
Community Services Coordinator Davis.
• October 8 th — Glenwood City van to Rice Lake
• November 16 — Baldwin & Woodville to Dunn County

A motion to accept the transportation requests was made by Supervisor Schrank and
seconded by Councilmember Burri; motion carried.
o Reconsideration of motion for Collaborative agreement between St. Croix Industries and
Aging Transportation to purchase a vehicle for 2010 per request of Supervisor Norton -
Bauman: Supervisor Norton- Bauman wanted to know if there was another way to fund
the purchase of the van for St. Croix Industries. Beckman spoke with David Lowe,
Wisconsin DOT, and Lowe agreed this collaboration would be within the guidelines of
the DOT Grant. The ADRC would own the van and lease it to St. Croix Industries. The
DOT 85.21 grant awarded to the ADRC annually requires that grantees provide services
to those of different abilities. There was discussion on St. Croix Industries being an
enterprise independent of the levy and the ADRC funding an enterprise. The goal of the
collaboration is to provide accessible transportation to individuals with disabilities in St.
Croix County. St. Croix Industries has the drivers and the ability to achieve this goal with
the purchase of a new accessible vehicle. The vehicle would be used mainly by St. Croix
Industries, but would be available for ADRC functions as well.
• Senior Center/Nutrition Program
• Senior Center Highlights Flu Shot Clinics were set up at each of the Senior Centers for
the month of October. The shots are being administered by Walgreens. Individuals can
call to make an appointment or walk -in.
The Monster Mash Bash is set for Friday, October 30 at the Roberts Park Building
from 1:00 p.m. to 3:00 p.m.
• Nutrition Program Managers Training_: Preventative/Nutrition Program Coordinator
Hanson informed the Council the Nutrition Program Managers and she will be attending
training in Rice Lake on October 8 th .
9. ADRC updates:
• Memory Walk September 19 Beckman informed the Council the ADRC will have a team at
the 2009 Memory Walk in New Richmond.
• Client Transition Update Beckman reported that in August, 11 individuals have chosen
Community Health Partnership — Partnership Plan, 51 have chosen Community Health
Partnership — Family Care Plan, 90 clients have been referred to IRIS with 82 enrolled and 18
individuals have completed Nursing Home Relocation. Ninety -two individuals from the
waiting list have been served and seventy -seven have been added to the list.
• ADRC Survg Options: Beckman informed the Council there will be students from St.
Scholastica Social Work program assisting with the formation of quality assurance surveys for
the ADRC.
