result in its premature failure to handle wastes. Proper
maintenance consists of pumping out the septic tank every three
years or sooner, if needed, by a licensed pumper. What you put
into the system can affect the function of the septic tank as a
treatment stage in the waste disposal system. Owner maintenance

of the St. Croix County Sanitary Ordinance.
The properly owner agrees to submit to St. Croix County
Planning & Zoning Department a certification form, signed by the
owner and by a master plumber, journeyman plumber, restricted
plumber or a licensed pumper verifying that (1) the on -site
wastewater disposal system is in proper operating condition and/or
(2) after inspection and pumping (if necessary), the septic tank is
less than 1/3 full of sludge
/we, the undersigned have read the above requirements and
agree to maintain the private sewage disposal system with the
standards set forth, herein, as set by the Department of Safety And
Professional Services and the Department of Natural Resources,

the three year expiation date.
1/we certify that all statements on this form are true to the best

INTERPRETIVE DETERMINATION REPORT FOR
GEORGE D & DORIS M. PETERSON TRUST
Site Address. 481290°F Street, Wilson
PARCEL If 004 —1019 — 60 - 00O
LOCATED IN THE NW Y.OF THE NW %, SEC. 09, T28N,
R15W, TOWN OF CADY, ST CROIX COUNTY, WI
This site has a completed soil evaluation which has determined that an
Interpretive Determination per Wisconsin Administration Code DSPS 383 is
required. The soils are mapped as SANTIAGO silt loam but determined by
this Soil Tester to be more like the FREEON series that is mapped near the
site with a 2% soil to the area and moderately well drained formed in deep
loess over dense sandy loam tills Attached is the series description. The A
horizon is a dark grayish brown with the subsoil being a brown This
indicates the A horizon does not stay saturated.
A mound system with 28 inches of ASTM C33 sand fill will handle the
hydraulic load and
treatment of the wastewater
