Chair Wentz called the meeting to order at 8:30 AM noting the meeting was
properly and legally publicized.

ANNOUNCEMENTS Johnson announced the Wisconsin Counties Association (WCA)
meeting is scheduled for April 15, 2009 from 6:00 to 8:00 PM. Johnson,
Whiting and Rebholz plan to attend.
Johnson displayed the current New Richmond Chamber of Commerce newsletter
which contains an article highlighting St. Croix Industries joining the
Chamber.
FINANCIAL REPORT The following financial report for Health and Human Services
through January, 2009 was distributed. The budget column of the following
report will be adjusted as changes in HRA insurance accounts were approved by
the County Board after the budget was approved, but the adjustments are
processed through the Government Center finance department. Pempek reported
Health and Human Services (HHS) advancements from the State are the reason for
the negative figure on the Administration line. Carryover monies will be
applied to the Administration line.

The following financial report for the Nursing Home through January, 2009 was
distributed. Adjustments will be made to the budget column of the following
report due to the changes in the HRA insurance accounts, which are the same
adjustments for Health and Human Services. The following report will be
modified to reflect one - twelfth of the $191,000 carryover funds to be applied
each month versus one lump sum at year end. IGT revenues will also be applied
to the current report. Johnson reported we will not receive any of the rate
appeal monies budgeted. Administrative services will adjust when the contracted
administrator position is filled by a county employee.

AGED RECEIVABLE REPORT: The aging Accounts Receivable report for Health and
Human Services through January was reviewed. Approximately $8,920 from 2006 was
removed from the books as it is no longer collectible from insurance.
Berenschot questioned how much is being written off compared to the amount
taken in. Pempek will get this figure. Write offs include reduced contractual
amounts, sliding fee scale amounts, and clients who cannot pay.
Pempek reported the Nursing Home auditors will be in -house next week.
Pempek has worked with Pietrick and Schroeder to set up St. Croix Industries as

a separate enterprise fund. Pempek reported the financial reports for ADRC
will change to show more detail in the future.
HEALTH AND HUMAN SERVICES PLANNING FOR OFFICE MOVES The facility went to a
single point of entry and locked all exterior doors except the front entrance,
where consumers are able to access the building. Staff have access to the
building with an ID badge. Interior and exterior signage was updated due to
this change. John Shilts from the Sheriff's Department has performed a security
assessment on the building and will present his findings at a future Board
meeting. Concerns were raised due to the distance from the front entrance to
Economic Support plus the ramp leaving Economic Support is an issue. There are
from 100 to 150 consumers that go to Economic Support every week. Many are
elderly, disabled or have young children.
To meet the needs of the consumers, first the ADRC area had to be addressed.
Three ADRC staff are temporarily in New Richmond while their Hudson suite is
being remodeled. The next planned step will be to move staff in New Richmond.
Staff who work more in the community versus seeing clients in -house will be
moved to the back of the HHS building and Economic Support staff will be moved
to the front of the building to address client access concerns. This move has
been coordinated by Ron Kiesler with Building Services and Information
Technology. The move will involve staff time and energy but is recommended to
meet the needs of consumers. Kiesler outlined some of the details of the
project, which is anticipated to involve minimal costs. Building Services and
Information Technology will be performing the modifications needed in each
unit. There is no timeline as to when the project will be completed. Johnson
will report back to the Board on costs for the project at the April meeting.
There have been discussions that have identified the cost of moving HHS staff
to be as much as $150,000. No costs for the project have been received at this
time, but as stated earlier, the expectation is the cost will not approach this
figure.
Tobin mentioned the transition so far has gone smoothly due in part to the help
of CIP inmates. He said CIP is an asset to our facility and will be issuing a
letter of appreciation to the Department of Corrections.
The Board recessed from 9:22 to 9:30 AM.
BEHAVIORAL HEALTH OUTPATIENT SERVICES PROGRAM REPORT Kiesler reported the
waiting list for Behavioral Health rose in January and February. He reported
it is fairly typical to have an increase at this time of year but the spike was
higher than normal and it could be a result of the current economy. Staff have
discussed the possibility of limiting access of insured clients to county
services as clients with insurance could be served by private vendors. Clients
with Medical Assistance and uninsured clients are not typically served by
private vendors. If services to insured clients were limited, revenues would
decline. Accounts Receivable staff are now reporting to HHS Department Heads
the status of waiting lists on a regular basis. The trend seems to be tapering
now so services will not be closed to any clients at this time.
Kiesler also reported that one psychiatrist who has clinic hours 14 hours /week
resigned effective 03/27/09. Staff are working to fill that psychiatrist time
with another vendor. Psychiatric intake has been closed at this time with the
exception of CSP referrals, adolescent referrals and court referrals/
commitments.

Kiesler reported mobile crisis has moved to 24/7 coverage as of 03/01/09 with
clinic staff providing coverage Monday through Friday from 8:00 AM to 4:00 PM.
Kiesler reported in 2006, 2007 and 2008 mobile crisis had been 75% effective in
diverting clients from in- patient hospitalization in crisis situations.
However, for January and February 2009 the percentage has dropped to 20 %. It
is not due to staff or policy /procedure; it is due to the population being that
difficult and it would not be safe to divert these clients. An additional step
of holding one bed for crisis at Atlas House has been implemented.
Kiesler reported county cell phone service has been reviewed with the help of
Dana Baker. Current services are with Sprint /Nextel and T- Mobile. Kiesler
said Baker continues to do an excellent job and has consolidated the current
phones into one plan which will save the county approximately $6,000 per year.
Kiesler also congratulated HHS Department Heads for their cooperation and
assistance with this consolidation. Johnson also recognized the work Kiesler
has done to consolidate the cell phone plans.
Kiesler reported Comprehensive Community Services (CCS) is slated to begin
04/08/09. This is a Medicaid reimbursable program for wrap around services to
adults and children. The program is not part of the WIMCR cap. Approximately
ten adolescent clients and one to two adult clients will be transferred to this
program from Youth Community Support Services (YCSS) once certified with more
to be added over time. It has taken a year to apply for this program. Kiesler
said we received approximately $40,000 from a State grant for this program last
year. He recognized the hard work and effort Huston, Campbell, Dunlap, Kirt,
and Markgraf have invested to implement this program that will benefit
residents of the county.
STATE DIVESTMENT POLICY FOR MEDICAL ASSISTANCE ELIGIBILITY Divestment is the
disbursement of assets for less than the fair market value with the intent of
receiving Medical Assistance. Previously people could give assets away and
after 36 months could apply for Institutional Medical Assistance and be
eligible. If the same people put assets into a trust they would have to wait
for 60 months to be eligible for Institutional Medical Assistance. The
ineligibility would start at the date of divestment.
The look back period is 60 months for any transfer trust or other transfer
with the new changes effective 01- 01 -09. The date of divestment now is the
date the client becomes eligible for Institutional Medical Assistance.
If the customer is ineligible for Institutional Medical Assistance they may
still be eligible for card services, which means they may see a doctor, go to
a hospital and /or get meds. They just will not be paid for nursing home
services; which will affect St. Croix Health Center. Nursing homes cannot
discharge residents if they have no place to send them; the nursing home
would have to provide the care needed with no revenue.
A transfer of income or assets is not divestment if the individual can
demonstrate that the transfer wasn't made with the intent of becoming
eligible for Medical Assistance.
A Life Estate in another individual's home is divestment unless they reside
in the home for a period of 12 consecutive months.
Promissory notes or loans can cause divestment.

Annuities purchased after 01 -01 -09 must designate the state as a beneficiary.
Divestment penalty period will be waived if it deprives the client of medical
care that the individual's health or life would be endangered or food,
clothing, shelter or other necessities of life.
Ineligibility occurs if the equity value of the home is greater than
$750,000.00
These new changes will increase the time staff spend on cases greatly. The
Nursing Home must look at how they will screen new residents to ensure
payment until Medical Assistance has been established. Squyer indicated this
is one more additional requirement from the State without added funding.
Rasmussen noted for - profit nursing homes will find a way not to take clients
who do not pay.
Squyer also noted there were 95 new cases in Economic Support in January and
February.
NURSING HOME ADMINISTRATOR'S REPORT Reiman reported the February census
average was 62 and the census today is 69, with an expected census of 70 by the
end of today. He reported of the payor mix there are nine Medicare A, the best
payor source, and most are therapy residents. There are also eight Medicare B
residents in the mix.
Reiman reported the tubs in the Nursing Home don't work well and are so old
parts are not readily available. Reiman was excited to report one of the
Building Services staff, Brian Cloutier, located and purchased two gently used
tubs for $400 each. Replacement tubs would cost approximately $13,000 each.
Building Services staff are installing the tubs today.
The surveyors were highly complimentary of the staff teamwork and quality of
care during the recent survey. He said no citations were received for the third
year in a row. He said with no citations in the Nursing Home, it is very
complimentary to the staff, Director of Nursing, and managers. Reiman also
mentioned it is rare that a 72 -bed nursing home have only one social worker for
that many residents and the county is very lucky to have Lisa Leahy filling
that position. Reiman congratulated staff on a job well done.
Johnson announced the Board would take a short recess to go to the dining room
to express appreciation to the Nursing Home staff for a job well done.
The Board recessed to the Nursing Home social room from 10:30 to 10:50 AM to
meet with Nursing Home staff.
2009 -2011 STATE BIENNIAL BUDGET REVIEW FROM WCA LEGISLATIVE STAFF Johnson
distributed a memo on the 2009 -2011 State Biennial Budget from Sarah Diedrick-
Kasdorf. He reported WCA (Wisconsin Counties Association) will lobby around HHS
(Health and Human Services) as a priority. He reminded the Board this document
is a proposal of the changes in the budget and the budget has not been
approved. He noted IGT for the Nursing Home may be funneled away from the
Nursing Home and there may be an increased bed tax. Johnson encouraged Board
members to thoroughly review the document. Johnson offered to department
specific forward information to Board members so they have opportunity to get
to legislators so we receive State budget relief and the burden doesn't fall as

heavily on the County budget.
MEDICAL REPRESENTATIVE APPOINTMENT TO THE HHS BOARD Dr. Lisa Ramsay has
offered her services to St. Croix County. Rebholz, Wentz, and Johnson have met
with her and were high impressed by Dr. Ramsay. She will be appointed to the
Health and Human Services Board at the April County Board meeting. Rebholz
announced Berenschot and Rasmussen will also be appointed at the April meeting
to continue their terms on the Health and Human Services Board.
PERSONNEL ISSUES NEW EMPLOYEES /VACANCIES Johnson announced Rachel Johnson
started as a Family Resource Aide in the Family and Children's Unit on March 8,
2009. He also announced Jody Kroening, RN, will begin as Behavioral Health
nurse on March 30. He said reference checks are being done and an offer will
be made soon for the open Medical Records position. He reported second
interviews and a tour of the facility will take place on March 23, 2009 for
the final candidates for the Nursing Home Administrator position. Public
Health nurse interviews will take place March 19, 2009.
TOBACCO -FREE GROUNDS RESOLUTION A copy of the proposed resolution was
forwarded to Board members. Wentz recommended delaying action on the
resolution until the State budget is implemented and we receive possible
assistance from the Governor's budget. Whiting said the expectation is that the
Governor's proposal for the Clean Indoor Act will pass.
Motion A motion was introduced by Peterson, seconded by Grant and adopted
with Berenschot voting no; That the Board approves tabling the tobacco -free
resolution as presented until after the Governor's budget is adopted.
Berenschot felt pressure should be kept on legislators to implement the bill.
Johnson indicated the actions of the Health and Human Services Board should not
impact the Governor's bill on Clean Indoor Act. Larson felt the resolution
should be tobacco free vs. smoke free and the Board should move forward
regardless of what the governor does.
Johnson indicated concern regarding the consumers at St. Croix Industries being
impacted and the resolution will also be brought forward at the County
department head meeting.
Peterson indicated the policy language needs to be cleaned up regarding
equipment and vehicles; smoke -free should be changed to tobacco -free, adding
government buildings, county -owned parks, all county vehicles and equipment,
etc. to read as follows:
RESOLUTION FOR A TOBACCO -FREE POLICY FOR COUNTY GROUNDS WITH GOVERNMENT
BUILDINGS, COUNTY -OWNED PARKS, THE FAIRGOUNDS
AND COUNTY -OWNED VEHICLES
Resolution No.
St. Croix County, Wisconsin
WHEREAS, the St. Croix County Board of Supervisors passed a resolution in
1992 that directed all County Government buildings and passenger vehicles be
smoke -free (with the exception of the day room area of the jail) effective
January 1, 1993; and

WHEREAS, the St. Croix County Board of Supervisors passed a resolution in May
of 2005 indicating:
1. The undersigned hereby requests that the Wisconsin Legislature
strengthen state laws to fully protect workers and the public
from the known health dangers of secondhand smoke by adopting
legislation that creates smoke -free workplaces.
2. The undersigned also requests that any state legislation or
regulation must preserve the right of local levels of government
to adopt more restrictive measures to protect citizens from
secondhand smoke; and
WHEREAS, the St. Croix County Department of Health and Human Services, at the
May, 2007 meeting, directed the Health and Human Services Director to send a
letter to State legislators indicating support of Senate Bill 150, also known
as the Breathe Free Wisconsin Act with the following considerations:
• Support a uniform Wisconsin Statute change to ban smoking to
include all indoor places but to exclude private passenger
vehicles and any private residence.
• Smoking ban to exclude outdoor areas except as currently defined
in Statute.
• That fines and penalties should not be increased; and
WHEREAS, the Surgeon General's Report of 2006, "The Health Consequences of
Involuntary Exposure to Tobacco Smoke," declares there is no safe level of
exposure to tobacco smoke; and
WHEREAS, the provision of tobacco free environments is an effective measure
of protecting the health of the employees of St. Croix County and that of the
public we serve; and
WHEREAS, the St. Croix County Department of Health and Human Services Board
met on January 30, 2008 to consider strengthening the County's existing
smoke -free policy; and
WHEREAS, the St. Croix County Department of Health and Human Services Board
selected an October 1, 2009 implementation date of a tobacco -free policy for
the St. Croix County Government grounds with Government buildings, including
the fairgrounds, County -owned parks and County -owned vehicles and equipment
(regardless of location).
WHEREAS, the Department of Health and Human Service staff will draft a plan
for the stages of implementation of the policies which will include, but not
be limited to, meetings with county building, county -owned parks and

THEREFORE BE IT RESOLVED that the St. Croix County Board of Supervisors
directs that all County Government grounds with Government buildings, County -
owned parks, the fairgrounds and all County -owned vehicles and equipment be
tobacco -free effective October 1, 2009.

REVIEW OF PUBLIC HEALTH PROGRAM REPORT AND LONG TERM SUPPORT PROGRAM REPORT
GIVEN TO COUNTY BOARD ON 03/16/09 Johnson reported Public Health and Long
Term Support gave a report at the March County Board session. This information
will be included in the annual report, which will be presented in May or June.
AD HOC FACILITIES PLANNING COMMITTEE UPDATE The next Ad Hoc Committee meeting
is scheduled for April 1 at 9:30 AM at Health and Human Services. Board
members were invited to tour the areas where the proposed HHS office moves will
take place in the near future. Kiesler will display a blueprint of the areas
that will be impacted.
Wentz asked if there were any other issues to bring before the Board.
