APPLICATION:
Applications will not be accepted until the applicant has:
• Met with the Zoning Administrator to review the application;
• Submitted Original pfus 2 copies of the entire packet including all supporting information, maps & diagrams;
• Resolved any land use violations and paid any outstanding lees owed to the Community Development Department;
• Signed the application form (the signatures of the property owners and oqents acting on their behalf are required); and
• Submitted the appropriate application fee (nonrefundable) payable to: St. Croix County.
REVIEW:
1 fie Zoning Administrator will review the application for completeness arid assign a file number to the application. The Zoning
Administrator may require additional information and will notify the applicant of this within 10 days. Upon receiving a complete
application and supporting documents, the Zoning Administrator will:
• send copies of the applications to the appropriate reviewing agencies for comment Applicants are encouraged to contact
their town to discuss their application and inquire about necessary building permits and approvals at the town level;
• schedule a site visit to the applicant's property, at which time the applicant shall flag all applicable property/project corners
and label the flags accordingly; and
• review the file and prepare findings for approval or denial of the permit within 60 days. Upon approval, the permit will be
mailed to the applicant or to the applicant's agent.
If approved, the land use permit will be valid for one year from the date of the permit issuance. The applicant may also be required
to apply through the Town lot a local building permit. All site plans, pictures, and other materials submitted with the application
become the properly of the Catnmunity Development Department arid will remain in the file.

❑ A general written statement that specifically explains the request.
❑ A statement indicating whether or not a private water or sanitary system is to be installed.
Recorded Warranty Deed (may be obtained at the Registerof Deed's office).
❑ A complete site plan showing:
• project location in the town;
• lot/parcel dimensions with total lot area, property lines, and all applicable setbacks;
location of existing access roads, right of way, road setbacks, and recorded easements;
• location of all existing and proposed structures with their square footage and distance Irotn setbacks;
• location of existing and proposed VOW TS, wells, driveways, parking areas, access, signs, and other features;
• location of slopes 12% and greater (minrmum contours to be determined by the Zoning Administrator);
• Al blulllines and slope preservation zones (Riverway) and setbacks from blufflines;
• location of the OHWM of any abutting navigable waterways and all setbacks from the OHWM;
• location and landward limit of all wetlands, specifications and dimensions for areas of proposed wetland alteration;
• existing and proposed topographic arid drainage features and vegetative cover;
• location of floodplain and floodway limits on the property; and
• any other unique limiting conditions of the property or information deemed necessary by the Zoning, Administrator.
For protects that involve land disturbance requests, the following, additional inlormation may be required:
❑ Detailed drawings (scale should not be greater than 1 inch to 200 feet).
❑ Grading plan showing grading Grnils and pre and post contours.
❑ Project schedule and contractor list.
❑ Erosion control plan (Rest Management Practices).
U Storm water management plan stamped by an engineer and including all runoff calculations.
❑ Vegetation plan including schedule, seeding rates, and species size, type and location.
❑ Other documents:

Community I)ceelopmenl Slaff have reviewed your Land Use Permil application for the construction of a new
acc"s0lA structure in the Shoreland Overlay District, and condilionallV approved the request as explained below.
Please note the following background information, as well as how the conditions relate to the issuance of this
permit. as the owner is respon,iblc for ensuring all conditions are met. Failure to meet the conditions niav result
in cnforcentew actions being taken and or this permit being revoked.
BACKGIZOIL ND INI'ORSIATION:
1. I he proposed structure will be located in the Down Glenwood.
The structure is proposed: beyond any applicable shoreline OI I\Vyl setbacks. greater than 50' loom any
wetland, more than 5- from the nearest side property line. and no filling grading or structures are proposed
within any mapped floodplain.
The proposal will disturb 2.000 tech within 1000' of the 01 MAI of the unnamed navigable waterbody.
All filling and grading activities will be conducted on slopes le„ than 25°rb.
1. ]'he proposed impervious surface located within the shoreland area will be less than 15% and therefore do
not require mitigation, per NR I 15 requirements.

accessory structure used for storage of personal itenn,
f INI)INGS AND CONDITIONS:
1. The new structure must meet the 1611owing setbacksrequirements when constructed:
• -?5' from the O\b'HN~1 ofa navigable water body''feature
• 5' side rear yard lot line
• 1 lie structure may not exceed 20' in height. I he height is meawred front the grade around the
structhue to the average height between the peak of the roof and caves.
• Cannot be located in any mapped floodplain without prior authorization from this department.
Frosion control must be implcmcnlcd according to the standards outlined in SCC 1 . 30(K)(2) for all filling
and grading activities.
a. No slopes in excess of 25°,o ina\ he disturbed, per SCC 17.12(10).
h. All disturbed areas on the site must be inunediatcfv planted with native vcgelation. as soon as
permissible in the building sequence. If planting inuncdialcly is not feasible and soils will remain
bite for more than one week, they shall be covered with erosion control blankets or a suitable means
al erosion control per accepted standards.
Before any ground disturbance takes place, the applicant or designated agent shall contact this department
for an onsite verification of erosion control measures, to make sure they are properly installed downslopc
from the proposed disturbance area. I'lie erosion control measures must be in place at the time this
departntcut is contacted for an onsite inspection. All erosion control shall be maintained to prevent sediment

plans provided. If deviations are going to occur from the plans, it most be approved by this deparhncnt.
.I. Erosion control meaurres shall not be removed until sufficient vegetative cmer has been established on
disturbed areas (approx. 7W re-vegelatcd). And this department gives permission for its removal.
AM driI c%way (if one is conslrucled or the existing access is going to be improved) most receive a permit
from the Tome and have appropriate erosion control installed prior to construction.
6. The impervious surfaces proposed are under Wo, and no mitigation is required per WI Admin. Code \R

If the location of ally proposed structures are altered or rev°ised from the plans included in this approval,
this department shall be :riven written documentation accompanied by plans which shove the desired location
of the structure. Written approval most be obtained before anv deviation from this approval occurs.
I his permit is valid for one vcar. with the possibilil} of up to two (2) six month extensions. It' extensions are
required, contact this department for details.

SO MAY RESULT IN A CITATION. At the time an inspection is desired, the Connnunity Development
Department shall be given at minimum 2 full business days' nolicc to ensure the it meeting time can be arranged
with a staff member.
This approval does not allow for any additional construction, structures or structural changes, grading.
filling. or clearing of vegetation beyond the limits of this request. Your information will remain oil file in
the tit. Croix County C'ommunily Development Department. It is your responsibility to ensure compliance
with any other local, state, or federal rules or regulations, including contacting the local Town to see if they
require any additional permits.
