Arrangements for inspection shall be made with the designated county official in accordance with the provisions ofSec. 145.20(2)(d), Wis. Stats. • Maintenance information must be given to the owner of the tank explaining the periodic cleaning of the effluent filter is required. Access to the filter for cleaning must be provided per Comm 84 product approval conditions. • Per directions of designer, effluent testing per Comm 83.44(2), Wis. Adm. Code shall be conducted on a yearly basis; (see additional comments of the management and contingency plan on page 10). Test results shall be submitted to the county for review. • A copy of the approved plans, specifications and this letter shall be on -site during construction and open to inspection by authorized representatives of the Department, which may include local inspectors. • This approval does not include plans for the general plumbing systems or sewer piping leading to the _ septic/holding tank that may be required for this project. Owners shall receive a copy of the appropriate operation and maintenance manual and/or owner's manual for the POWTS described in this approval. • The owner of a POWTS shall be responsible for ensuring that the operation and maintenaice of the POWTS occurs in accordance with this chapter and the approved management plan under s.Comm 83.54(1). • In the event this soil absorption system or any of its component parts malfunctions so as to create a health hazard, the property owner must lbllow the contingency plan as described in the approved plans. • The owner is responsible for submitting a maintenance verification report acceptable to the county for maintenance tracking purposes. Reports shall be submitted at intervals appropriate for tie component(s) utilized in the POWTS. In granting this approval the Division of Safety & Buildings reserves the right to require changes or additions should conditions arise making them necessary for code compliance. As per state stats 101.12(2), nothing in this review shall relieve the designer of the responsibility for designing a safe building, structure, or component. Tank inspections must include a visual inspection of the tank(s) to identify any missing or broken hardware, identify any cracks or leaks, measure the volume of combined sludge and scum and to check for any back up or ponding of effluent on the ground surface. The dispersal cells) shall be visually inspected to heck the effluent kmeb in the observation pipes and to check for any ponding of effluent on the ground surface. The ponding of effluent on the ground surface may indicate a failing condition and requires the Immediate notification of the local regulatory authority. When the combined accumulation of sludge and scum in any tank equals one -third (Y,► or more of the tank volume, the entire contents of the tank shag be removed by a Septage Servicing Operator and disposed of in accordance with chapter NR 113, Wisconsin Administrative Code. All other services, including but not limited to the servicing of effluent filters, mechanical or pressurized components, pretreatment units, and any servicing at intervals of 512 months, shag be performed by a certified POWTS Maintainer. Here's a summary: • The soil report is extremely close to the 100 -year floodplain elevation (1071.94') contour shown on the site plan, so Shaun will need to obtain elevations for the test pits using Mean Sea Level per Comm 85.40(3)(a) 5.e. The POWTS can't be installed in a floodplain, period. Code also requires the CST to provide the surface elevation for the river, even though the tested area is over 150' from the river OHWM. We'll schedule a county on -site to verify site /soil conditions and elevations with Shaun, but need to have the floodplain elevation staked. We do have reference elevations on the bridge deck, but the walkout elevation on the building is --10 ft. above grade at B3 and over 300' away and it would be helpful to have a visual "limit" for where the POWTS can /cannot be installed. • The CSM Outlot will have to change before any structures (like parking lot) can be built on that property. Peace Lutheran should consider working with Alex Blackburn on combining the parcels into one and removing the "outlot" status. The POWTS may be OK to install, since it technically isn't a structure and was included in the SE Permit's filling & grading approval. • The driveway fire lane shown over the top (between septic vents) of the existing POWTS that services the parsonage must be eliminated. Move it elsewhere, but not within 15' of any POWTS on the site. The older septic systems will hopefully be OK with the location of the southerly fire lane ... future replacement will have to be west of the fire lane in the `butlot" area. • The grading plan notes for the stormwater structure mention an emergency overflow elevation at 1072.0' and the top of the berm at 1072.5'. The overflow is reinforced and directs discharge out the west side of the basin —100' south of the POWTS area (measured from 63). I'm just concerned that the main discharge pipe is only about 50' away from the septic area, so we may require a condition in the sanitary permit to install a swale between the stormwater pond and B3 to direct surface water away from the POWTS. He will need to redo all profile descriptions after onsite verification is completed. It would be advisable to do an onsite because of the high wastewater flow, flood plain issues, and adjacent soils with shallow depth to LS BR. I also want to know what the line is below the Cty Rd N. Is it the edge of the pavement, the right -of -way, or the centerline? He should also give the distance from that point to the centerline of the NS road or to the property line to tie the BM location down. I have been instructing people for years to give this information so there should be no argument from Shaun in this regards. The RFE needs to be reported by Shaun on his report. In such cases he needs to tie his BM into the MSL datum or simply use MSL elevations. Hopefully, someone has an elevation on one of the existing buildings or a nearby bridge. In addition, and if practical, he needs to give the elevation of the adjacent river. All vents and unsealed manholes must be at least 2.0 ft above the RFE. This is an outlot. The following message addresses the sanitary permit issuing agent's (SPIA) role in such reviews. Reviews by the department are outlined in Tables 83.22 -1 and 83.22 -2. Systems located in the floodplain are not included in the tables. An "Application for Development of a Flood Plain" (SBD -6698) form (formally known as a Plb. 89 form) is no longer required by the department for development within the floodplain. When a system is to be located in a floodplain area the following code sections need to be considered: 1. Comm 83.32(3)©, Wis. Adm. Code states that no part of a POWTS may be installed in a floodway except as provided in NR 116, Wis. Adm. Code. 2. Comm 83.45(6)(a), Wis. Adm. Code requires POWTS treatment and holding tanks to be watertight. And (6)(b) requires vent and observation pipes to terminate 2 feet above the regi flood elevation. 3. Comm 83.43(8)(g), Wis. Adm. Code requires POWTS trea meet an o i ponents installed in areas subject to soil saturation to be anchored to prevent flotation. Comm 85.4 3 a e Wis. Adm.
