Fourth Street New Richmond, Wisconsin 54017 - 1063 LONG TERM SUPPORT PLANNING COMMITTEE MEETING MINUTES NOVEMBER 3, 1999 MEMBERS PRESENT: Robert Boche, Keith Gregerson, Fern Johnson, Ray Mealey, John Mortensen, Richard Peterson, Francis Schaffner, Marguerite Sumner, Glenda Zielski, Wanda Viellieux, Robert Palewicz MEMBERS EXCUSED: Bruce Bayer, James Craig MEMBERS ABSENT: STAFF PRESENT: Ron Lockwood, Social Work Supervisor; Sandra Gehrmann, Aging Department Director CALL TO ORDER: Meeting called to order by chair Ray Mealey ROLL CALL: Eleven present and two excused. ADOPT AGENDA: Motion by Fern Johnson; second by Marguerite Sumner to accept the agenda for today's meeting as submitted; carried. PREVIOUS MINUTES ACTION: Motion by Wanda Viellieux to accept the previous meeting minutes as written; second by Keith Gregerson; carried. NEXT MEETING: Thursday, February 10, 2000 at 1:30pm. Human Services complex in New Richmond; 74 Board Room. UNFINISHED BUSINESS: CY2000 LTS Unit Budget - Budget was passed by the County Board as presented, for both LTS and Aging Departments Home Modifications Policy - (See attachment) Motion to accept policy as presented by Robert Palewicz; second by Richard Peterson; motion carried. NEW BUSINESS: Financial Report- (see attachment) If CIP2 is underspent, the money is lost. Chemically dependent category is zero because the primary diagnosis is probably something else. Workforce shortage is causing problems in Long Term Support also. Moving people out of Northern Center provides more choice and quality for them. Much discussion revolved around this issue. Variance & Purchase Requests: None Aging Unit Report: (By Sandra Gehrmann) State Budget items affecting seniors; additional funds for AFCSP, independent living centers, family care pilots, transportation, and 581 additional COP slots. Advocacy for Federal incentives Advocacy for state congregate and home -delivered meal program funds. Home care needs will continue to be documented by various agencies. Elderly pro-bono project was described. Motion to adjourn by Fern Johnson, second by Marguerite Sumner; carried Recorded by Sandy Gehrmann, Director Department on Aging I10199 Il 59 PM ST CROIX COUNTY LONG TERM SUPPORT UNIT Policy on Home Modifications DEFINITION Home modifications are those services which assess the need for, arrange for and provide modifications and/or improvements to a participant's residence to allow for community living and to ensure the health, safety, security. accessibility and the maximum degree of independent functioning. Such adaptations may include the installation of ramps and grab bars, widening of doorways, modification of bathroom facilities, or installation of specialized electric and plumbing systems which are necessary to accommodate the medical equipment and supplies necessary for the welfare of the individual. Excluded are those adaptations or improvements to the home which are of general utility, and are not of direct medical or remedial benefit to the individual, such as carpeting, roof repair, central air conditioning, etc. Additions, which add to the total square footage of the home, are excluded from this benefit. SERVICE REQUIREMENTS/LIMITATIONS/EXCLUSIONS l . General House repairs (roof repair, plumbing) are not allowable unless the necessary repair is of a previous Waiver allowable modification. 2. Home modification funds are intended to respond to accessibility and related needs associated with the person's disabilities and are not intended to cover basic construction costs. I No waiver funds may be expended for home modifications if the MA card can cover the modification. 4. Housing modification services are provided to meet the needs that have been identified as part of the care plan to prevent institutionalization. 5. The Long Term Support Coordinator or the LTS Social Work Supervisor shall approve all housing modifications over $2,000 purchased under this policy. 6. The Long Term Support Coordinator must approve all purchases over $15,000. 7. The Director of the Health and Human Service Dept must approve purchases over $20,000. 8. On projects over $5,000 at least two bids/proposals will be sought, unless further delay would impose an unreasonable hardship on the participant, as determined by the Human Service Director. STANDARDS 1. Providers must meet state or local requirements for licensure or certification (such as building contractors, or engineers) if required. 12:59 PM 2. All improvements/modifications must be made in accordance with local and state housing and building codes. 3. Approval will be made only under conditions where the modifications have little or no utility apart from an individual's disability, and where the individuals with the physical or cognitive disabilities would not otherwise be able to reside in the home. OCUMENTATION 1. An itemized list of any home modifications must be included in the Individual Service Plan that is approved by the Wisconsin Department of Health and Family Services. 2.
