The ad that was published is as follows: St. Croix County Health and Human Services is seeking bids for approximately 8,000 gallons of 87 octane unleaded gas and approximately 14,000 gallons of diesel fuel of #2 grade vehicle fuel and arctic blend diesel vehicle fuel to be delivered throughout 1998. Please submit sealed bids to St. Croix County Health and Human Services, 1445 North Fourth Street, New Richmond, WI 54017. Bids accepted until 12:00 noon on Friday, January 3, 1998. Any questions, contact Rich Monn between 7:30 a.m. to 4:00 p.m. Monday through Friday at 715/246-8210. We reserve the right to refuse any and all bids. Stephens stated he read all bids. Skogland Oil was chosen as they could furnish the arctic blend diesel vehicle fuel. Stephens stated that Home Oil Company was asked to bid but declined due to a conflict of interest. MOTION: A motion was introduced by Stephens, seconded by Bethke, and unanimously adopted: To accept the bid received from Skoglund Oil. PERSONNEL ISSUES: Stephens introduced June Merkt, RN, who has been hired as our new Home Health Supervisor, replacing Marcey Backes, RN. Merkt began employment on January 5, 1998. HEALTH AND HUMAN SERVICES/NURSING HOME REPORT: Krieger stated the State survey team is here. They came on Wednesday and the exit interview is today at 1:00 p.m. Krieger invited all Board members to attend. Bethke asked where do the nursing home referrals come from. Krieger stated that the majority of the referrals come from the hospitals and it depends on the discharge planners. The patients now have to be informed of every place available that could meet their needs. Ultimately the family is the ones who decide what facility their family member will go to. Krieger stated the nursing home has three types of admissions: 1) Residents needing Aggressive Therapy - so they can be discharged home, 2) Terminal - here until they die, and 3) Alzheimer residents - no one at home to take care of them. Stephens stated the turnaround at mealtime in the nursing home used to take around 2 hours. Now everyone gets fed at the same time so there is a quicker turn around. Stephens stated this is a success story. The residents are given tremendous attention at mealtime. All employees work equally hard. Everyone in the dining room is attending to the residents. Bethke asked about the elevator - we used to have problems with it and haven't heard anything lately. Borup stated we have changed to another company. This company is very responsive to 7 any problems. even on weekends. We have less of a problem than two years ago and at a better cost. We are r Ttting our monies worth. Borup stated last week we received very good news. We received Medical Assistance deficit reduction dollars: Home Health - $85,000 for 1996; Mental Health/Alcohol and Other Drug Abuse, Community Support Program, and Long Term Support - $86,000; Public Health - $3,700. This was a windfall for us. This was more money than we had anticipated. LONG TERM SUPPORT PRESENTATION: Hall and Lockwood gave a presentation on the Community Options Waiver (COP W) and Community Integration Program II (CIP II). Because these two programs serve the same population (elderly and physically disabled) and operate under the same rules, they were referred to as one program: CIP II/COP W. These programs are also referred to as Home and Community Based Waivers, which is the Federal Medicaid term for them. (CIP II/COP W is an agreement or waiver with the Federal Medicaid agency to use Medicaid funding for services in the recipient's home in the community rather than in an institution.) County participation in these waiver programs was mandated effective January 1, 1990, and all counties are actively participating. Individual service plans are developed for each applicant by the appropriate county agency and submitted for approval to the state as required by federal regulations. Each service plan is reviewed to ensure that the proposed plan of care meets all federal specifications, is comprehensive, individualized, and guarantees the health, safety and welfare of the program participant. The state contracts with an outside vendor, The Management Group, Inc., to monitor these safeguards and compliance with program requirements by county agencies. Hall's presentation was to increase knowledge of CIP II/COP W waivers waiting list. Hall stated often times that the first referral comes from the hospital when client is being discharged. They may go to a nursing home and wait for an opening in CIP II/COP W. Prior to clients waiting for an opening in CIP II/COP W their needs may be met temporarily by relatives or a short nursing home stay. Hall followed an actual case from waiting list through assessments, case plan, approval and audit. Hall stated there are three assessments that need to be completed to determine what level of care the client is at: 1) functional assessment, 2) financial assessment and, 3) medical assessment. The functional screen is the eligibility determination before they can be put on the waiting list. The assessment findings are then sent to the State and the State reviews the plan. The plan needs to be approved before the plan goes into effect. The case manager has to make contact with the client at least one time a month, and every three months there has to be a face-to-face contact. Home stated because of this presentation he understands that the peoples' needs are being met while they are on the waiting list and that was a concern of his. Stephens wanted clarity, is anyone suffering physically or mentally because they are not on the waiting list. Hall stated that is not in black and white. 9 Lockwood dis,--ibuted copies of brochure "What is the difference between COP, CIP, and COP- W?" This is a detailed description of the programs. The Board requested that at the March 10`h meeting Hall and Lockwood return and continue with the eligibility criteria. Also how do you get on the waiting list, how many are there on the waiting list and when they were referred, etc. Stephens thanked Hall and Lockwood for the presentation. Kinship Care Report: Effective 01/01/98, all open AFDC -NLRB (Non -Legally Responsible Relatives) cases were transferred from Economic Support to Family and Children's Unit for Kinship Care Program. Johnson gave a brief overview of caseload size, program requirements and funding. This is not an intense program but does have six-month and annual reviews. The family receives $215.00/month per child. Johnson stated Family and Children's utilizes relative placement. Johnson stated they try keeping the children with family whenever possible. Borup stated this program is a by-product of W-2 which mandated the change of program administration from Economic Support to Family and Children's. Johnson stated that many counties have found the funding from the State has short falls. Some counties are going to pay until the State money runs out, and then implement a waiting list. Johnson stated that State Representative said you couldn't have a waiting list for Kinship cases, as this is an entitlement program. However, the State is aware that some counties intend to implement a waiting list for this service if State allocations do not cover costs. The needs of the program would not be fulfilled if there were a waiting list. The Board asked that St. Croix County not start a waiting list for this program. Johnson stated this is a very worthwhile program. It keeps the family together to avoid other types of placement. Johnson stated we always look at relative's history, criminal checks, child support checks, etc. We look at each on a case -by -case basis. The Health and Human Services Board asked that Johnson update them in three months. Stephens asked that Johnson update the Board at the April 1998 meeting. County money allocated to this program for the 1998 budget will remain. Domestic Abuse Funding Changes: Borup stated that for many years we have received $6,000. Recent changes in State funding will result in counties losing domestic abuse funding across Wisconsin. Our loss will be $6,000. The State will establish new programs "through a competitive bidding process". This change also means that our County "matching" funds is no longer required. Borup stated it was approved in the budget and would like to continue it. We will wait to hear from Madison, as we do not know right now. Borup stated he would like to continue the manager's 5-minute report on three program areas to help keep the Board abreast of programs on a monthly basis. The Long Term Support Unit will report at the March meeting and at the April meeting there will be an update of the Kinship Care Program. 9 Stephens asked if there were any other issues to bring before the Board. Bethke stated Schottler has done a wonderful job in clearing the land. He is satisfied and enjoys working with us. Berenschot stated there would be a meeting on Healthy Community and Healthy Youth at the Community Center Sunday night, 01/18/98 at 6:00 p.m., meal included. If interested, let Berenschot know. He has a brochure to view. He stated the more community assets for the children, the better the kids do.
