Fourth Street New Richmond, Wisconsin 54017 -1063 TO: St. Croix County Long Term Support Planning Committee FROM: Donna Patterson, L.T.S. Case Manager PIP DATE: October 27, 10-8 RE: Spending Limit for Chore Services I am requesting a variance on the policy which places a $3,000.00 annual limit for chore services. My client, Tom Brown, is 17 years old and severely disabled as a result of a motor vehicle accident two and one-half years ago. He continues to require full assistance in all activities of daily living. Tom lives in a single parent household with his mother who works full time. The BIW has been paying for weekly cleaning services. Without this service, his mother would need to spend more time cleaning and have less time available to assist and socialize with her son. The cost of the cleaning services is $75.00 a week with a total annual cost of $3,900.00. I appreciate your consideration of this matter. If you have any questions or require further information, feel free to contact me at 246-8249. DP:mah CC: Ron Lockwood x ST. Motion to accept previous meeting minutes as submitted made by Marguerite Sumner, second by Keith Gregerson. Motion carried. Next meeting date set as January 21, ,1999 at 1:30pm in the board room of the Human Services Complex in New Richmond. NEW BUSINESS: Ron Lockwood reported that the Human Services Board requests a discussion at their next meeting regarding this committee. At this Its com - meeting November 12, 1998 2 meeting the human services board would like to discuss the role of this committee and its relationship with the board. Ron asked if there were any members who would be able to attend this meeting. Several members including Keith Gregerson, Marguerite Sumner, Robert Boche, John Mortensen, Ray Mealey, Glenda Zielski, Robert Palewicz and Ron Lockwood stated that they would be able to attend. Ron will get approval for a per diem for those who attend. A notice will be sent as a reminder. This meeting will be held Tuesday, November 24 at 6:30pm in the board room of the Health & Human Services Complex. A discussion followed about possible questions and what the by-laws of this committee are. Ron handed out a copy of the by-laws to refresh everyone. UNFINISHED BUSINESS: 1999 LTS Budget: Budget requests were approved. Day Activities is not defined very well yet, but will have several young adults very severely physically disabled. These persons will be out of school and have no structured services. In the past St. Croix Industries has served well, but these persons are so severely disabled that SCI is not accessible enough or have the staff and space to provide the needed services of this population. As soon as a plan is developed for this, the money approved for this will be released. COP Start Up Grant Application: Ron Lockwood reported that the proposal is ready to send in to apply for funding for this grant. If the money is not used for a social worker position, we can use the money for COP Services as defined by the COP Program. Motion made to approve submission of proposal for COP Startup Funds made by Robert Palewicz, second by Keith Gregerson. Motion carried. Supportive Home Care Spending Limit Policy: Discussion regarding this policy and what criteria to use when approving or denying requests. The discussion was lengthy. A member brought up that this subject had been discussed previously and it was decided that the $3,000 annual limit was sufficient. A variance request for this spending limit is on the agenda today, but after more discussion, a motion was made by Wanda Viellieux to table this pending more information directly from the family or the person needing the assistance, second by John Mortensen. Motion carried. This item will be placed on the agenda for the next meeting to discuss issues of what criteria should be used to determine approval or denial of these requests. Financial Report: Ron Lockwood reported that figures look good and we are within budgeted amounts at this time. Guest: Dawn Marrilot is a nursing student from Century College in North St. Paul. Paul. She is here as part of her training following along with a senior citizen to see just how life is for them. She is here with Marguerite Sumner.
